Rhabdomyolysis secondary to zolpidem-induced seizures

Resumo: Introduction: Rhabdomyolysis is a condition in which muscle destruction and necrosis occurs. Of varied etiology, it may be associated with convulsive crises. Zolpidem is a non-benzodiazepine sedative and hypnotic agent that under conditions of abuse and frequency can cause seizures. Case Report: Male, 26-year-old man was taken to the emergency room at a general hospital due to anxiety, where he had episodes of seizures. The condition progresses to rhabdomyolysis. History of zolpidem abuse. Discussion: High doses of zolpidem alter the selective agonism in the benzodiazepine-1 receptor and the abrupt withdrawal of the medication can lead to seizures with evolution, in severe cases, to rhabdomyolysis. Conclusions: Self-medication and abusive use of substances such as zolpidem have the potential to cause tolerance and dependence and may lead to life-threatening clinical conditions.

De variada etiología, puede asociarse a crisis convulsivas. Zolpidem es un agente sedante e hipnótico no benzodiacepínico que, en condiciones de abuso y frecuencia, puede causar convulsiones. Caso Clínico: Un hombre de 26 años fue llevado a urgencias de un hospital general por ansiedad, donde presentó episodios de convulsiones. La condición progresa a rabdomiolisis. Antecedentes de abuso de zolpidem. Discusión: Dosis altas de zolpidem alteran el agonismo selectivo en el receptor de la benzodiazepina-1 y la retirada brusca de la medicación puede dar lugar a convulsiones con evolución, en casos graves, a rabdomiólisis. Conclusiones: La automedicación y el uso abusivo de sustancias como el zolpidem tienen el potencial de causar tolerancia y dependencia y pueden conducir a condiciones clínicas potencialmente mortales.Introdução: Rabdomiólise é uma condição em que ocorre a destruição e necrose muscular. De etiologia variada, pode estar associada a crises convulsivas. O zolpidem é um agente sedativo e hipnótico não benzodiazepínico que sob condições de abuso e frequência pode ocasionar crises convulsivas. Relato de Caso: Homem, 26 anos, foi levado à emergência de hospital geral por ansiedade, onde apresentou episódios de crises convulsivas. Quadro evolui para rabdomiólise. História de abuso de zolpidem. Discussão: Altas doses de zolpidem alteram o agonismo seletivo no receptor benzodiazepina-1 e a retirada abrupta da medicação podem levar a crises convulsivas com evolução, em quadros graves, para rabdomiólise.
