Resumo: | Objective: To detect, through the analysis of the historical evolution of antidepressants (ADs) and their putative mechanisms of action, if there is a scientific basis that guides their clinical use. Method: analysis of relevant clinical studies and systematic reviews published over the last sixty years on the mechanisms of action and clinical use of ADs, their relationship with diagnostic classification systems in psychiatry, the role of regulatory agencies and the evolution of the system current patents. Results: it is not possible to justify with the available clinical-scientific data the clinical choice of this or that group of ADs. Conclusion: Open, double-blind, randomized clinical studies do not lend themselves to choosing the clinical use of a given AD. As “meta-analyses” are based on these studies, their results are conflicting. The choice of an AD continues to be based on the psychiatrist's experience. |

Conclusión: Los estudios clínicos aleatorizados, abiertos, doble ciego, no se prestan a la elección del uso clínico de un determinado DA. Como los “metanálisis” se basan en estos estudios, sus resultados son contradictorios. La elección de un AD sigue basándose en la experiencia del psiquiatra.Objetivo: Detectar através da análise da evolução histórica dos antidepressivos (ADs) e de seus putativos mecanismos de ação se há base cientifica que oriente seu emprego clínico. Método: análise de estudos clínicos e de revisões sistemáticas relevantes publicadas nos últimos sessenta anos sobre os mecanismos de ação e emprego clínico de ADs, sua relação com os sistemas de classificação diagnóstica em psiguiatria, com o papel das agências reguladoras e com a evolução do sistema de patentes vigentes.Resultados: não é possível justificar com os dados clínico-cientificos disponíveis a escolha clínica desse ou daquele grupo de ADs. Conclusão: Estudos clínico abertos, duplo-cegos randomizados não se prestam à escolha do emprego clínico de um dado AD. Como “metanálises” são baseadas nos referidos estudos seus resultados são conflitantes.
