Resumo: Ekbom’s Syndrome, also known as delusional parasitosis, is a rare psychiatric disorder characterized by the patient's firm belief of being infected with parasites that emerge through the skin, typically from the scalp, genital area, eyes, or mouth. In an attempt to remove the imaginary worms, some patients may self-inflict skin injuries and engage in self-mutilation. Initial consultations often involve dermatologists who frequently propose dermatological treatments that are ineffective. Once diagnosed by a psychiatrist, treatment typically involves the use of antipsychotic medications, with Pimozide being the most commonly reported drug in the literature. Due to the rarity of this disorder, the objective of this study is to report a case of Ekbom’s Syndrome in an elderly female patient with delusional persecutory disorder, who achieved complete remission of symptoms after combined use of Olanzapine and Risperidone, thereby regaining her quality of life and sense of well-being. This work is based on a database that encompasses case reports, literature reviews, journal articles, pharmacology sources, and psychiatry manuals.

En un intento de eliminar los gusanos imaginarios, algunos pacientes pueden autoinfligirse lesiones en la piel y automutilarse. Es común que los pacientes consulten inicialmente a dermatólogos, quienes a menudo proponen tratamientos dermatológicos que no son efectivos. Una vez diagnosticado por un psiquiatra, el tratamiento generalmente incluye el uso de medicamentos antipsicóticos, siendo la Pimozida el fármaco más mencionado en la literatura. Debido a la rareza de este trastorno, el objetivo de este estudio es informar sobre un caso de Síndrome de Ekbom en una paciente anciana de sexo femenino con trastorno delirante de tipo persecutorio, que logró una remisión completa de los síntomas después del uso combinado de Olanzapina y Risperidona, recuperando así su calidad de vida y sensación de bienestar. Este trabajo se basa en una base de datos que abarca informes de casos, revisiones de literatura, artículos de revistas, fuentes de farmacología y manuales de psiquiatría.A Síndrome de Ekbom, também conhecida como delírio de parasitose, é uma doença psiquiátrica rara, caracterizada pela sólida crença do paciente de estar infectado por parasitas que saem pela pele, quase sempre, do couro cabeludo, região genital, olhos ou boca. Na tentativa de retirar os vermes, alguns pacientes removem fragmentos da própria pele, produzindo lesões e automutilações. É comum a procura inicial por dermatologistas, os quais, frequentemente, propõem tratamentos dermatológicos que não são resolutos. Quando diagnosticada por um psiquiatra, institui-se o tratamento por meio de antipsicóticos, sendo a Pimozida o fármaco mais relatado na literatura. Em razão da raridade desse transtorno, este estudo tem por objetivo relatar um caso de Síndrome de Ekbom em paciente idoso, do sexo feminino e com Transtorno Delirante do tipo persecutório, que apresentou remissão completa dos sintomas após uso associado de Olanzapina e Risperidona, retomando a qualidade de vida e sensação de bem-estar.
