Approach in the excoriation disorder: a developing field

Resumo: | Excoriation Disorder (ET), that is, the habit of causing skin lesions, usually on the face, causing functional impairment, is common in psychiatric psychopathology, but still infrequent in clinical practice. Less than 20% of patients with ET seek treatment. In this work, a bibliographic review was carried out on the proposed theme and possibilities of established approaches. The treatment of ET with Habit Reversal Therapy (HRT) is considered effective. This therapy is based on four techniques: awareness, competitive response practice, motivation and generalization. The main psychoactive drugs evaluated were Selective Serotonin Reuptake Inhibitors (SSRIs), with controversial results. Another approach that also addresses the reversal of habits is the Rothbaum Protocol (RP), which associates stress management with management. Unlike HRT, PR addresses co-occurring depressive and anxious symptoms. There is still some disagreement regarding the treatment of Excoriation Disorder. Therapies with greater applicability show better results for changing intrinsic habits. Psychotropic drugs for impulse control corroborate with greater awareness and, consequently, avoidance of symptoms. |

Menos del 20% de los pacientes con TE buscan tratamiento. En este trabajo se realizó una revisión bibliográfica sobre el tema propuesto y posibilidades de abordajes establecidos. El tratamiento de la TE con Terapia de Reversión de Hábitos (TRH) se considera eficaz. Esta terapia se basa en cuatro técnicas: conciencia, práctica de respuesta competitiva, motivación y generalización. Los principales psicofármacos evaluados fueron los inhibidores selectivos de la recaptación de serotonina (ISRS), con resultados controvertidos. Otro enfoque que también aborda la reversión de hábitos es el Protocolo Rothbaum (PR), que asocia el manejo del estrés con la gestión. A diferencia de la TRH, la PR aborda los síntomas depresivos y ansiosos concurrentes. Todavía hay cierto desacuerdo con respecto al tratamiento del trastorno de excoriación. Las terapias con mayor aplicabilidad muestran mejores resultados para el cambio de hábitos intrínsecos. Los medicamentos para el control de los impulsos corroboran con una mayor conciencia y, en consecuencia, evitación de los síntomas.O transtorno de escoriação (TE), ou seja, hábito de ocasionar lesões de pele, geralmente na face, causando prejuízo funcional é algo frequente na psicopatologia psiquiátrica, porém ainda pouco frequente na prática clínica. Menos de 20% dos pacientes com TE procuram tratamento. Neste trabalho, foi realizada revisão bibliográfica sobre o tema proposto e possibilidades de abordagens estabelecidas. O tratamento da TE com Terapia de Reversão de Hábitos (TRH) é considerado eficaz. Esta terapia é baseada em quatro técnicas: consciência, prática de resposta competitiva, motivação e generalização. Os principais psicofármacos avaliados foram os Inibidores Seletivos da Recaptação de Serotonina (ISRS), apresentando resultados controversos. Outra abordagem que também aborda a reversão de hábitos é o Protocolo de Rothbaum (RP), que associa o gerenciamento do estresse na gestão. Ao contrário da TRH, a RP aborda sintomas depressivos e ansiosos concomitantes. Ainda há alguma discordância em relação ao tratamento do Transtorno de Escoriação. Terapias com maior aplicabilidade apresentam melhores resultados para mudança de hábitos intrínsecos. Os psicotrópicos para controle dos impulsos corroboram com maior consciência e, consequentemente, evitação dos sintomas.
