CAPS distribution in the state of Paraíba, PB, Brazil: quantitative analysis

Resumo: Introduction: Psychosocial Care Centers are the main component of the extra-hospital network within the mental health care reform. They are an important instrument within territorial and community-based assistance.  Objectives: to map and analyze the distribution of Psychosocial Care Centers and their multidisciplinary teams in the state of Paraíba, based and focus on regionalization. Methods: a descriptive, cross-sectional study with a quantitative approach. Data from the Mental Health Coordination of the state of Paraíba – from the National Register of Health Establishments and Human Resources – were used. Coverage indices were calculated for the 16 regional health departments. Ordinances 336/2002 and 130/2012 from the Ministry of Health served as the basis for the analysis regarding the number of professionals per service. Results: Paraíba has 113 Psychosocial Care Centers distributed in the modalities: I, II, III, ad, ad III, and i. It was observed that the 1st region had the lowest coverage index (1,03), and the 4th region had the best result (5,74), both differing considerably from the total state index (2,12). Regarding the multidisciplinary teams, it was observed that the number of nurses (201) and other health professionals (448) numerically supplies the number of services, but that the number of doctors (60) is much lower than expected. It is worth mentioning that three regions do not have a registered psychiatrist on their staff, emphasizing the 12th region, which appears without any medical professional despite the existence of four services in its territory. Conclusions: The distribution of CAPS in Paraíba is wide and satisfactory; however,  few psychiatrists are working in these services.

Son un instrumento importante dentro de la asistencia territorial y comunitaria. Objetivos: realizar un mapeo y el análisis de la distribución de los Centros de Atención Psicosocial y sus equipos multidisciplinarios en el estado de Paraíba, a partir del enfoque de la regionalización. Métodos: estudio descriptivo, transversal con enfoque cuantitativo. Se utilizaron datos de la Coordinación de Salud Mental del estado de Paraíba, del Registro Nacional de Establecimientos de Salud y Recursos Humanos. Se calcularon tasas de cobertura para las 16 regiones de salud. Las Ordenanzas 336/2002 y 130/2012 del Ministerio de Salud sirvieron como base para los análisis sobre el número de profesionales por servicio. Resultados: Paraíba cuenta con 113 Centros de Atención Psicosocial distribuidos en las siguientes modalidades: I, II, III, ad, ad III e i. Se observó que la 1ª región tuvo el índice de cobertura más bajo (1,03) y la 4ª región obtuvo el mejor resultado (5,74), ambas muy diferentes del índice total del estado (2,12). En cuanto a los equipos multidisciplinarios, se observó que el número de enfermeros (201) y otros profesionales de la salud (448) suple numéricamente el número de servicios, pero el número de médicos (60) es muy inferior al esperado. Cabe señalar que tres regiones no cuentan con psiquiatras registrados en su plantilla, destacándose la 12ª región que aparece sin ningún profesional médico a pesar de la existencia de cuatro servicios en su territorio. Conclusiones: La distribución de los CAPS en Paraíba es amplia y satisfactoria, sin embargo, con un número reducido de psiquiatras actuando en estos servicios.Introdução:Os Centros de Atenção Psicossocial são o principal componente da rede extra-hospitalar dentro da reforma da assistência em saúde mental. Formam um importante instrumento dentro do atendimento com base territorial e comunitária. Objetivos: realizar mapeamento e análise da distribuição dos Centros de Atenção Psicossocial e de suas equipes multiprofissionais no estado da Paraíba, a partir do enfoque da regionalização. Métodos: estudo descritivo, de recorte transversal, com abordagem quantitativa. Foram utilizados dados da Coordenação de Saúde Mental do estado da Paraíba, do Cadastro Nacional de Estabelecimentos de Saúde e Recursos Humanos. Foram calculados os índices de cobertura para as 16 regionais de saúde. As portarias 336/2002 e 130/2012 do Ministério da Saúde serviram de base para as análises referentes à quantidade de profissionais por serviço. Resultados: A Paraíba possui 113 Centros de Atenção Psicossocial distribuídos nas modalidades: I, II, III, ad, ad III e i. Observou-se que a 1ª região apresentou o menor índice de cobertura (1,03) e a 4ª região obteve o melhor resultado (5,74), ambas diferindo bastante do índice total do estado (2,12). Em relação às equipes multiprofissionais, observou-se que a quantidade de enfermeiros (201) e de outros profissionais de saúde (448) supre numericamente a quantidade de serviços, mas que o número de médicos (60) é bem inferior ao esperado. Vale destacar que 3 regionais não possuem nenhum psiquiatra registrado em seus quadros funcionais, com destaque para a 12ª região que aparece sem nenhum profissional médico apesar da existência de 4 serviços em seu território.
