High depression prevalence in patients from a reference outpatient in fibromyalgia

Resumo: Introduction: Fibromyalgia (FM) is a chronic painful syndrome, which frequently co-occurs with depression, but the indexes of this co-occurrence are inconsistent among studies, showing large variability. Objective: Investigating and comparing the depression prevalence in patients coming from FM service of “Conjunto Hospitalar de Sorocaba“ with patients without FM from primary care. As well as, to evaluate age, socioeconomic status, employment, and being user of mental health service among these populations. Method: It was a cross-section study where were evaluated 35 women with FM and 27 women from primary care. Depression was assessed by the Beck Depression Inventory, for the others variables we used a sociodemographic questionnaire. The FM sample was mostly female, so we chose to exclude the men from the analysis. We compared the samples results according to sample provenience, and lately we evaluated only the FM sample, when we performed an exploratory correlations analysis, then the FM sample was separated in with depression and without depression to explore potentials differences among variables. Results: The depression prevalence observed was 65,7% in the FM sample and 29,6% in the without FM sample. Besides, the FM women were older (53,6 years older and 40 years older respectively), with more retirement/home stay rates, lower socio-economic indexes, and making previous use of mental health service, compared to the no FM. Conclusion: Given the high prevalence of depression and in our results, we could suggest that all patients diagnosed with fibromyalgia should be systematically screened to depression, aiming early diagnoses and interventions.

Objetivo: investigar y comparar la prevalencia de depresión en pacientes de la clínica de fibromialgia del “Conjunto Hospitalario de Sorocaba” sin FM, atendidos en el dispensario; así como evaluar edad, índice socioeconómico y uso de servicios de salud mental por parte de dicha población. Método: se llevó a cabo un estudio transversal donde fueron evaluadas 35 mujeres de la clínica de FM y 27 mujeres atendidas en el dispensario. Para determinar la presencia de depresión fue utilizado el Inventario de Depresón de Beck. Para las demás variables, se elaboró un cuestionario de datos sociodemográficos.Fueron comparados los resultados de las muestras de acuerdo a su origen. La muestra con FM era mayoritariamente compuesta por pacientes del sexo femenino, razón por la cual se optó por excluir pacientes de sexo masculino del análisis. Posteriormente, se evaluó la muestra con FM de forma aislada, cuando realizamos un análisis exploratorio de correlación y posteriormente dividimos en FM con depresión y FM sin depresión para explorar posibles diferencias entre las variables. Resultados: la prevalencia de depresión observada durante el periodo de estudio fue de 65,7% en la muestra FM y de 29,6% en la muestra dispensario. Las pacientes con FM eran significativamente de mayor edad (53,6 años y 40 años respectivamente), presentando mayor proporción entre aquellas de ocupación pensionista y ama de casa, con menor índice socioeconómico y que hacían más uso de los servicios de salud mental cuando comparadas con las pacientes del dispensario. Conclusión: dada la alta prevalencia del trastorno depresivo en el presente estudio, puede sugerirse la necesidad de que todos los pacientes de la clínica de fibromyalgia sean sistemáticamente investigados para confirmar o descartar la presencia de depresión, en un esfuerzo por realizar diagnóstico e intervención precoces.Introdução: Fibromialgia (FM) é uma síndrome dolorosa crônica e, frequentemente ocorre concomitantemente com depressão, mas as taxas dessa co-ocorrência são inconsistentes entre os estudos, com grande variabilidade. Objetivo: Investigar e comparar a prevalência de depressão em pacientes do ambulatório de fibromialgia do Conjunto Hospitalar de Sorocaba (FM) e em pacientes sem fibromialgia atendidas na unidade básica de saúde (SFM). Assim como avaliar idade, índice socioeconômico, emprego e uso de serviços de saúde mental nessas populações. Método: Estudo transversal onde foram avaliadas 35 mulheres com FM e 27 SFM. Para avaliação da presença de depressão utilizamos o Inventário de Depressão de Beck e para as demais variáveis utilizamos um questionário de dados sociodemográficos. Como a amostra com FM foi majoritariamente composta por mulheres, optamos por excluir os homens das análises. Comparamos os resultados das amostras de acordo com a origem e posteriormente avaliamos apenas a amostra com fibromialgia, quando realizamos uma análise de correlação exploratória e subsequentemente dividimos em FM com depressão e FM sem depressão para explorar as possíveis diferenças entre as variáveis. Resultados: A prevalência de depressão observada no período do estudo foi de 65,7% na amostra FM e de 29,6% na SFM. As pacientes com FM eram significativamente mais velhas (53,9 anos e 40 anos), com maior proporção de mulheres aposentadas e do lar, com menor índice socioeconômico e com maior uso de serviço de saúde mental, quando comparadas às SFM.
