Child abuse and neglect are serious global problems and may be in the form of physical, sexual, emotional, or simply neglected problems in meeting the child's needs. Such factors can leave the victim with permanent psychological damage in the same way that, if not detected by the authorities, can lead to early death. It is considered a priority issue in public health, being even more marked in childhood. Information on disease morbidity may be undersized due to obstacles in case detection.?In the present case report, a 2-year, 8-month-old boy was physically assaulted by a close relative who caused a real physical and emotional trauma to the boy. After satisfactorily managing the traumas and emotional effects of such a child, in addition to counseling services provided to the caregiver, the patient continues to be accompanied by neuropediatrics and related, and is constantly monitored. He was also referred to a child support group for social support, given the hostile environment in which he lived. The authors' objectives are to draw the attention of other health professionals to observe carefully the cases that initially appear to be irrelevant; many times the victim arrives with an initial complaint of domestic accident, if there is a critical approach of the professional who receives this type of case, several patients will not have their problems solved and can evolve to death. Finally, with this publication, we intend to demonstrate the contrast of the magnitude of violence against the child, measured actively in an outpatient clinic, with the spontaneous casuistry of the service.
