Dificuldades para implantação do protocolo de cirurgia segura na América do Sul: uma revisão sistemática

Resumo: | Every surgical procedure has risks, which vary according to the complexity of the surgery and the profile of the patient. However, there are risks that can, and must, be avoided. For that, there are many tools, one of which is the World Health Organization: Safe Surgery Checklist. The goal of this paper is to evaluate the main difficulties regarding the use of the Checklist in South America and to propose tactics to enhance its application. This is a systematic review written according to the PRISMA recommendations and using as scientific database the following: Scientific Eletronic Library Online (SciELO), Latin American Literature in Health Sciences (LILACS), PubMed and Science Direct. Articles raging from January 2010 to April 2021 were analyzed. 19 articles, from Argentina and Brazil, were included in the final sample, which showed as the item with worst adhesion “Time Out” in 55% (n=6), followed by “Sign In” with 27% (n=3). The health professionals pointed to lack of knowledge of the Checklist, lack of compliance and the existence of a surgery hierarchy as barriers to the efficient application of the instrument. For this reason, administrators of health teams have the fundamental role in stimulating the participation of the whole staff, increasing the quality of communication and capacitation of the affiliates and as a result ensure safer surgeries. |

Entretanto, existem riscos que podem, e devem, ser evitados e para isso existem diversas ferramentas, sendo uma delas o checklist de cirurgia segura da organização mundial da saúde. Este trabalho objetiva avaliar as principais dificuldades na aplicação do checklist nos países da América do Sul e sugerir abordagens para o aprimoramento de sua utilização. Trata-se de uma revisão sistemática realizada segundo as recomendações propostas pelo PRISMA e cujas bases de dados científicos utilizados foram: Scientific Eletronic Library Online (SciELO), Literatura Latino Americana em Ciências da Saúde (LILACS), PubMed e Science Direct. Foram analisados artigos publicados entre janeiro de 2010 até abril de 2021. 19 artigos, provenientes de Argentina e Brasil, foram incluídos na análise, os quais evidenciaram como item de pior adesão o “Time Out” em 55% (n=6), seguido pelo “Sign In” com 27% (n=3). Os profissionais de saúde apontaram falta de conhecimento quanto ao checklist, má adesão da equipe e existência de uma hierarquia cirúrgica como barreiras na implantação eficaz da ferramenta.
