Análise radiográfica, tomográfica e microtomográfica dos efeitos do alendronato no reparo ósseo de fraturas: estudo em fêmur de ratos

Resumo: | Bone repair is a complex and well-orchestrated physiological process typically characterized by overlapping stages: initial inflammatory response/soft callus formation, hard callus formation, initial bone union and bone remodeling. The aim of this work was to evaluate the effects of sodium alendronate in bone repair of fractures. Thirty-six Wistar rats were divided into three groups: group C (control), group B1 and group B2. The rats underwent femoral fracture surgery with stable internal fixation with plate and screws of the 2.0 system. The animals received intraperitoneal applications of alendronate sodium at a dose of 1mg / kg (B1) and 3mg / kg (B2) three times a week, while the control group received 0.9% physiological solution. The applications lasted 45 days, until the moment of euthanasia, in which the femurs were removed and sent for radiographic, tomographic and microtomographic analysis. Data were submitted to descriptive and inferential statistical analysis (95% CI). Quantitative evaluations of bone neoformation did not show differences among the groups in the radiographic (p=0.341) microtomographic (p=0.581) and in the tomographic evaluations (p=0.171). In the qualitative microtomographic analysis it was possible to observe a smaller distance between the internal bone trabeculae in the groups that used alendronate (p=0.05). In addition, group B2 had a higher amount of bone trabeculae per unit length when compared to the other groups (p=0.04). It can be inferred that the use of the drug did not have a direct influence on the amount of bone neoformation, but it influenced, in a dose-dependent way, the bone quality, having effects on the distance and quantity of trabeculae. |

O alendronato é um medicamento que inibe a reabsorção óssea, a qual constitui estágio fundamental no reparo de fraturas. O objetivo deste trabalho foi avaliar os efeitos do alendronato de sódio no reparo ósseo de fraturas. Foram utilizados 36 ratos da linhagem Wistar divididos em três grupos: grupo C (controle), grupo B1 e grupo B2. Os ratos foram submetidos a cirurgia de fratura do fêmur com fixação interna estável com placa e parafusos do sistema 2.0. Os animais receberam aplicações intraperitoneais, três vezes na semana, de alendronato de sódio na dose de 1mg/kg (B1) e 3mg/kg (B2), enquanto o grupo controle recebeu aplicações de solução fisiológica 0,9%. As aplicações tiveram duração de 45 dias, até o momento da eutanásia, em que os fêmures foram removidos e enviados para análise radiográfica, tomográfica e microtomográfica. Os dados foram submetidos a análise estatística descritiva e inferencial (IC 95%). As avaliações quantitativas de neoformação óssea não apresentaram diferenças entre os grupos tanto na avaliação radiográfica (p=0,341) como na avaliação microtomográfica (p=0,581) e tomográfica (p=0,171). Já na análise qualitativa microtomográfica foi possível observar uma menor distância entre as trabéculas ósseas internas nos grupos que faziam uso de alendronato (p=0,05). Além disso, o grupo B2 apresentou maior quantidade de trabéculas ósseas por unidade de comprimento quando comparado aos outros grupos (p=0,04).
