Avaliação de simetria mamária em pacientes mastectomizadas pelo aplicativo BSymmetry

Resumo: Background: Search for satisfactory aesthetic outcomes related to quality of life without major psychological damage to the patients represents one of the major challenges to the treatment of breast cancer. Aims: (1) Literature review; Systematic review of existing literature on the evaluation of cosmetic as well as breast symmetry in patients who underwent surgeries for the treatment of breast cancer. With research in PubMed with the following descriptors: aesthetic mammary, breast symmetry, mastectomy, bcct.core; of the last ten years. (2) This study aims to evaluate and compare the BSymmetry app, through a similarity study with BCCT.core software. Methods: (1) Systematic review of the existing literature on the evaluation of cosmetic as well as breast symmetry in patients who underwent surgeries to treat breast cancer. (2) This is a unicentric, retrospective, cross-sectional analytical study with the description of its clinical records and patient records. All patients were submitted a nipple or skin sparing mastectomies, with immediate reconstruction, in the Nossa Senhora das Graças Hospital, Breast Unit. Photographic documentation was done in the pre- and postoperative period. Breast symmetry analysis was performed using the BSYMMETRY app and BCCT.core software. Results: There was a reasonable degree of agreement between the BSYMMETRY and BCCT.core methods, evidenced in both pre and postoperative evaluations, with kappa = 0.29 in both; as in the evolution of breast symmetry, kappa = 0.26. Conclusions: (1) Lack of a gold standard method for evaluation of breast symmetry. Description of existing methods to evaluate mammary cosmesis and description of a new method for analysis of breast symmetry. (2) The BSYMMETRY application showed reasonable agreement and / or similarity when compared to BCCT.core software.

(2) The BSYMMETRY application showed reasonable agreement and / or similarity when compared to BCCT.core software.Introdução: A busca por resultados estéticos satisfatórios mantendo uma qualidade de vida sem maiores danos psicológicos à paciente, representa um dos maiores desafios no tratamento do câncer de mama. Objetivos: (1) Revisar a literatura. (2) Avaliação e comparação do aplicativo BSYMMETRY, através de estudo de similaridade e concordância com o software BCCT.core. Métodos(1) Revisão sistemática da literatura existente sobre a avaliação da cosmética, bem como, da simetria mamária em pacientes que foram submetidas a cirurgias para tratamento do câncer de mama. Com pesquisa na PubMed com os seguintes descritores: estética mamária, simetria mamária, mastectomia, bcct.core; dos últimos dez anos. (2) Tratase de um estudo unicêntrico, retrospectivo, observacional, analítico; de análise de arquivos fotográficos e prontuários médicos de pacientes submetidas a mastectomias nipple ou skin sparing e reconstrução imediata com prótese. Todas as cirurgias foram realizadas no Serviço/Unidade de Mastologia do Hospital Nossa Senhora das Graças pela mesma equipe. Foram feitas documentações fotográficas no pré e no pós-operatórios. A análise da simetria mamária foi realizada utilizando o app BSYMMETRY e o software BCCT.core. Resultados: Houve um grau de concordância razoável entre os métodos BSYMMETRY E BCCT.core, evidenciado tanto nas avaliações pré e pós-operatórias, com kappa = 0,29 em ambos; quanto na evolução da simetria mamária, kappa = 0,26. A simetria mamária se manteve inalterada em 54,68% das pacientes operadas. Conclusões: (1) Inexistência de um método padrão ouro para avaliação de simetria mamária. Descrição de métodos existentes para avaliar cosmese mamária e, descrição de um novo método para análise de simetria mamária.
