Resumo: Atopic dermatitis (AD) is a highly pruritic chronic inflammatory disease of multifactorial etiology. About 85% of patients have AD for the first time before the age of 5 years and 20-40% remain with manifestations of the disease until adulthood. An important factor for the evolution of IgE-mediated pathologies is dysbiosis in the intestinal microbiome, by inducing sensitization through systemic immunological and inflammatory reactions. The use of probiotics to modify the microbiota and, then, achieve relevant effects in organs away from the intestine, such as the skin, is a known technique, however, it is controversial among the authors. The aim of this study is to identify scientific evidence on the benefits of probiotics in the prevention or treatment of patientswith AD. This is an integrative literature review based on scientific articles available in PubMed and Scielo databases using the keywords "Atopic dermatitis" AND "gut microbiome", "Atopic dermatitis" AND "probiotics" and "Atopic dermatitis" AND “gut microbiome” AND “probiotics.” We selected 9 articles, published in the last 6 years (2017-2022), that meet the interests of the study. Based on this, research has shown that L. rhamnosus, L. plantarum, L. reuteri and L. pentosus strains of probiotics were ableto decrease the Atopic Dermatitis Scoring, a score related to AD severity, and even reduce the risk of its development. Furthermore, the heat-inactivated L. paracasei CBA L74 strain proved to be beneficial for reducing the use of corticosteroids in patients, minimizing their adverse effects due to their excessive use and improving adherence to conventional therapy. However, other studies have obtained conflicting conclusions, which may be related to the influence of factors such as the type of strain, age of the individuals, dose and duration of treatment, which may affect the effectiveness of the application of probiotics for this purpose. Prevention or therapy with probiotics in AD appears to be favorable. However, this topic still needs more research that proves the true role of probiotics and can elucidate their mechanisms of action in the intestinal microbiome of AD patients, aiming at a better quality of life for these individuals.

Cerca de 85% dos pacientes apresentam a DA pela primeira vez antes dos 5 anos de idade e 20-40% permanecem com manifestaçõesda doença até a fase adulta. Um fator importante para a evolução das patologias mediadas por IgE é a disbiose no microbioma intestinal, por induzir a sensibilização através de reações imunológicas einflamatórias sistêmicas. A utilização de probióticos para modificar a microbiota e, então, conseguir efeitos relevantes em órgãos afastados do intestino, como a pele, é uma técnica conhecida, porém, conflituosa entre os autores. O objetivo deste estudo éidentificar evidências científicas sobre os benefícios dos probióticos na prevenção ou no tratamento de pacientes com DA. Trata-se de uma revisão integrativade literatura realizada a partir de artigos científicos disponíveis nas bases de dados PubMed e Scielo através dos descritores "Atopicdermatitis”AND “gut microbiome”, "Atopicdermatitis” AND “probiotics” e "Atopicdermatitis”AND “gut microbiome” AND “probiotics”. Foram selecionados 9 artigos, publicados nos últimos 6 anos (2017-2022), que atendem aos interesses do estudo. Com base nisso, as pesquisas demonstraram que as cepas L. rhamnosus, L. plantarum, L. reuterie L. pentosusde probióticos foram capazes de diminuir o Scoring Atopic Dermatitis, pontuação relacionada à gravidade da DA, e até reduzir o risco do seu desenvolvimento. Ainda, a cepa L. paracasei CBA L74 inativada pelo calor se mostrou benéfica para a redução do uso de corticosteroidesnos pacientes, minimizando seus efeitosadversos por sua utilização excessiva e melhorando a adesão à terapia convencional.Porém, outros estudos obtiveram conclusões conflitantes, que podem estar relacionadas à influência de fatores, como o tipo da cepa, idade dos indivíduos, dose e duração dotratamento, podendo afetar a eficácia da aplicação dos probióticos nesse intuito. A prevenção ou terapêutica com probióticos na DA aparenta ser favorável.
