Associação entre ΔOHIP-14 e genes ANKK1, DRD2, ESR1, ESR2 e FKBP5 em pacientes classe III submetidos a cirurgia ortognática

Resumo: Introduction: Dentofacial deformities (DFD) are skeletal discrepancies associated with malocclusion, which negatively affect aesthetics, oral function, personality and social behavior. These deformities can be corrected through orthognathic surgery, which, together with orthodontic treatment, is an ideal and safe method. Some factors may predispose the improvement or worsening of the quality of life (QoL) of patients in orthognathic surgery, such as sex and genetic factors. Objective: To evaluate the preoperative association and OHIP-14 variation (ΔOHIP-14) in the perception of quality of life related to oral health (OHRQoL) with sex and genetic polymorphisms. Materials and methods: This study included 99 adult patients, class III, of both sexes, who underwent orthognathic surgery. All patients were assessed preoperatively, one week before surgery, and 6 months after surgery. The Oral Health Impact Profile questionnaire (OHIP-14) was applied, and oral mucosa cells were collected for DNA extraction. The investigated genes were ANKK1 (rs1800497), DRD2 (rs6275 and rs6276), ESR1 (rs2234693 and rs9340799), ESR2 (rs1256049 and rs4986938) and FKBP5 (rs1360780 and rs3800373). And they were genotyped by the polymerase chain reaction technique in real time (PCR-RT). The results obtained were submitted to statistical analysis, with a significance level of 0.05. Results: In the preoperative evaluation, women showed worse perception when compared to men for the total OHIP-14 (p = 0.004) and for domains psychological discomfort (p = 0.005), psychological disability (p = 0.009) and social disability (p = 0.018). There was no association between sex and ΔOHIP-14 in any of the domains (p > 0.05). In the preoperative period we found an association with rs1800497 related to physical pain and rs1256049 with the functional limitation domain. In ΔOHIP-14, it was possible to observe associations of the genetic polymorphisms rs1800497, rs6275 and rs6276 with the functional limitation. It was also possible to observe that the ANNK1 gene is related to total OHIP-14 and ESR2 to the physical pain domain (p < 0.05). Conclusion: Class III women have a worse perception of OHRQoL in the preoperative period. Polymorphisms in the ANKK1, DRD2 and ESR2 genes are involved with the perception of QoL in the preoperative period and in ΔOHIP-14.

No entanto, alguns fatores podem predispor a melhora ou piora da qualidade de vida (QV) dos pacientes submetidos a cirurgia ortognática, como o sexo e fatores genéticos. Objetivo: Avaliar a associação pré-operatória e OHIP-14 variação (OHIP-14) na percepção da qualidade de vida relacionada a saúde bucal (QVRSB) com o sexo e polimorfismos genéticos dos genes ANKK1, DRD2, ESR1, ESR2 e FKBP5. Materiais e métodos: Foram incluídos nesse estudo 99 pacientes adultos, classe III, de ambos os sexos, que foram submetidos a cirurgia ortognática. Todos os pacientes foram avaliados no pré-operatório, uma semana antes da cirurgia, e no pós-operatório de 6 meses da cirurgia. Foi aplicado o questionário Oral Health Impact Profile (OHIP-14) e realizado a coleta de células da mucosa bucal para extração do DNA. Os genes investigados foram ANKK1 (rs1800497), DRD2 (rs6275 e rs6276), ESR1 (rs2234693 e rs9340799), ESR2 (rs1256049 e rs4986938) e FKBP5 (rs1360780 e rs3800373). E foram genotipados pela técnica de reação em cadeia da polimerase em tempo real (PCR-RT). Os resultados obtidos foram submetidos à análise estatística, com nível de significância de 0,05. Resultados: Na avaliação pré-operatória, as mulheres demonstraram pior percepção quando comparadas aos homens para o OHIP-14 total (p = 0,004) e para os domínios desconforto psicológico (p = 0,005), incapacidade psicológica (p = 0,009) e na incapacidade social (p = 0,018). Não houve associação entre o sexo e o ΔOHIP-14 em nenhum dos domínios (p > 0,05). No pré-operatório encontramos associação com rs1800497 relacionado a dor física e o rs1256049 com o domínio limitação funcional. Já no ΔOHIP-14, foi possível observar associações dos polimorfismos genéticos rs1800497, rs6275 e rs6276 com a limitação funcional. Também foi possível observar que o gene ANNK1 está relacionado com o OHIP-14 total e o ESR2 com o dominío dor física (p < 0,05). Conclusão: As mulheres classe III possuem uma pior percepção da QVRSB no pré-operatório.
