Resumo: INTRODUCTION: Rosai-Dorfman Disease (DRD), also known as Sinus Histiocytosis, is a benign and rare histiocytic disorder of unknown etiology. Clinically, in most cases, it manifests as a massive and painless lymphadenopathy, especially in the cervical region. Extranodal involvement has been documented in approximately 40% of cases. The use of [18F]FDG on PET/CT is a well established tool in the diagnosis, staging and follow-up of malignant neoplasms. However, there is a great expansion of the indications of this method, as it is very effective and highly sensitive in the follow-up of patients with benign diseases, such as Rosai Dorfman's disease. OBJECTIVES: This study aimed to report a clinical case of RosaiDorfman Disease with extranodal involvement. It was described the clinical presentation of the disease, histological characteristics, diagnosis and treatment. The use of PET/CT in the followup of the disease was discussed, thus defending the benefits of its use. For this purpose, it was intended to highlight more updated cases in which such examination was a useful tool for the diagnosis and staging of the disease. MATERIALS AND METHODS: The work developed is a descriptive and observational study, of the case report type, which described an unusual case of Rosai-Dorfman's Disease, accompanied at the Paraiba Oncology Center (João Pessoa, Paraíba), with data obtained through medical records. RESULTS AND DISCUSSION: This is a relevant study for presenting an unusual case of Rosai-Dorfman disease, which reinforces the need for implementation of new modalities of diagnosis and staging to the future protocols of DRD, which will help in a more careful decision of the best approach for the treatment of the disease. The PET/CT proved to be an appropriate tool to perform the initial evaluation of the extension of the disease, as well as the response to the therapy instituted, also avoiding patients from being submitted to excessive and often ineffective procedures. CONCLUSION: The present study reported an unusual case of Rosai-Dorfman disease with extranodal involvement, in which PET/CT was used, helping in the treatment decision. Clinicamente, na maioria dos casos, se manifesta como linfadenopatia maciça e indolor, principalmente em região cervical. Acometimento extranodal foi documentado em aproximadamente 40% dos casos. O uso da [18F]FDG no PET/CT é uma ferramenta já bem estabelecida no diagnóstico, estadiamento e acompanhamento de neoplasias malignas. Entretanto, percebe-se uma grande expansão das indicações desse método, por apresentar bastante eficácia e alta sensibilidade no acompanhamento de pacientes portadores de doenças benignas, como é o caso da Doença de Rosai Dorfman. OBJEITVOS: O presente estudo visou relatar caso clínico da Doença de Rosai-Dorfman com acometimento extranodal. Descreveu-se a apresentação clínica da doença, características histológicas, diagnóstico e tratamento. Discutiu-se o uso do PET/CT no acompanhamento da doença, defendendo assim os benefícios do seu uso. Para este propósito, buscou-se evidenciar casos mais atuais em que tal exame foi ferramenta útil para o diagnóstico e estadiamento da doença. MATERIAIS E MÉTODOS: O trabalho desenvolvido é um estudo descritivo e observacional, do tipo relato de caso, que descreveu caso incomum da Doença de Rosai-Dorfman, acompanhado no Centro Paraibano de Oncologia (João Pessoa, Paraíba), com dados obtidos por meio de prontuário médico. RESULTADOS E DISCUSSÃO: Trata-se aqui de um estudo relevante por trazer caso incomum da Doença de Rosai-Dorfman, que reforça a necessidade de implementação de novas modalidades de diagnóstico e estadiamento aos futuros protocolos da DRD, que irão auxiliar numa decisão mais criteriosa da melhor abordagem para o tratamento da doença. O PET/CT se mostrou ferramenta apropriada para realizar a avaliação inicial da extensão da doença, bem como a resposta à terapia instituída, poupando também os pacientes de serem submetidos à procedimentos excessivos e muitas vezes ineficazes. CONCLUSÃO: O presente estudo relatou caso incomum da Doença de Rosai-Dorfman com acometimento extranodal, em que o PET/CT foi utilizado, auxiliando na decisão do tratamento.
