Relationship between timely clamping of the umbilical Cord and cases of pathological jaundice

Resumo: Introduction: The practice of performing the timely clamping of the umbilical cord, although recommended by the main means of neonatal reference and considered a good obstetric practice, is debated in several literatures regarding its risks and benefits, which question its relation with cases of pathological jaundice Thus, the study aimed to evaluate the relationship between timely clamping of the umbilical cord and cases of pathological jaundice in the first days of life. Method: Analytical, cross-sectional observational study with retrospective analysis of a quantitative nature. We sought to analyze data on births assisted by obstetric nursing at a public hospital in the Federal District in the year 2021 through the unit's record book. Result: The study did not show a statistically significant relationship between the occurrence of cases of pathological jaundice and delayed cord clamping. The occurrence of more severe cases of jaundice related to blood incompatibility was observed in all cases. Among these, the most severe cases of jaundice had primiparous mothers and male new borns as a striking feature. Conclusions: The occurrence of cases of hyperbilirubinemia with in 24 hours of life related to delayed clamping did not show statistical significance, being with in the expected margin for the development of jaundice cases. On the other hand, a direct relation ship was observed between severe cases and ABO incompatibility between mothers and new borns.

On the other hand, a direct relation ship was observed between severe cases and ABO incompatibility between mothers and new borns.Introdução: A prática de realização do clampeamento oportuno do cordão umbilical ainda que preconizada pelos principais meios de referência neonatal e tida como uma boa prática obstétrica é debatida em diversas literaturas quanto aos seus riscos e benefícios as quais questionam sua relação com os casos de icterícia patológica. Desta forma, o estudo objetivou avaliar a relação entre o clampeamento oportuno do cordão umbilical e casos de icterícia patológica nos primeiros dias de vida. Método: Estudo do tipo observacional analítico, transversal com análise retrospectiva de natureza quantitativa. Buscou-se analisar dados de nascimentos assistidos pela enfermagem obstétrica de um hospital público do Distrito Federal do ano de 2021 por meio do livro de registros da unidade. Resultado: O estudo não mostrou relação estatística significativa entre a ocorrência de casos de icterícia patológica e o clampeamento tardio do cordão. Foi observado a ocorrência de casos mais graves de icterícia relacionados a incompatibilidade sanguínea na totalidade dos casos. Entre estes, os casos mais graves de icterícia tinham como característica marcante mães primíparas e recém-nascidos do sexo masculino. Conclusões: A ocorrência de casos de hiperbilirrubinemia com 24h de vida relacionada ao clampeamento tardio, não mostrou relevância estatística, estando dentro da margem esperada de desenvolvimento dos casos de icterícia.
