Osteoarthritis of the TMJ

Resumo: Osteoarthritis (OA) is a very recurring degenerative disease, which is correlated with tension and mechanical load imposed on the temporomandibular joint (TMJ) and has bilateral involvement in most cases. Therefore, the knowledge of the professional, dental surgeon, about the main etiological factors, as well as the diagnostic criteria, is of paramount importance, aiming at an effective therapy for the assisted patient. The present study aimed to better understand the triggering process of osteoarthritis in the temporomandibular joint. In the methodology used, in the selection of the sample, criteria were defined for articles indexed from 2016 to 2021, published in national and international journals, available in full in Portuguese and English languages. Data collection was performed using only the Google database, since it provides all the articles necessary to prepare the current study. The following descriptors: osteoarthritis; ear-jaw articulation and treatment were used. The search resulted in 50 articles after using the inclusion and exclusion criteria; 17 articles remained for analysis, interpretation and discussion. In the results, it was evident that the differential diagnosis of osteoarthritis must be made in relation to rheumatoid arthritis, myofascial pain and TMJ dysfunction syndrome, and various forms of internal TMJ disorders. Several inflammatory and degenerative mechanisms act together to produce the signs and symptoms and pathophysiological characteristics of TMJ Osteoarthritis.

A identificação e o controle dos processos de degeneração da articulação temporomandibular, ainda são um grande problema para a Odontologia, em vista dos sintomas serem bastante parecidos com outros distúrbios temporomandibulares. Diante disso é de suma importância o conhecimento do profissional, cirurgião dentista sobre os principais fatores etiológicos, assim como os critérios diagnósticos, visando uma terapêutica eficaz para o paciente em tratamento. O presente estudo teve como objetivo compreender melhor o processo desencadeador da osteoartrite na articulação temporomandibular. Na metodologia empregada, na seleção da amostra definiram-se critérios de artigos indexados de 2016 a 2021, publicados em periódicos nacionais e internacionais, disponibilizados na íntegra em língua portuguesa e inglesa. A coleta de dados foi realizada usando somente a base de dados Google haja vista que ela fornece todos os artigos necessários para elaborar um trabalho. Os seguintes descritores foram utilizados: osteoartrite; articulação temporomandibular; diagnóstico e tratamento. A busca deu como resultado inicial 50 artigos científicos. Entretanto, após a utilização dos critérios de inclusão e exclusão; restaram 15 artigos para análise, interpretação e discussão. Nos resultados, ficou evidente que o diagnóstico diferencial da osteoartrite deve ser feito em relação com a artrite reumatóide, com a síndrome da dor e disfunção miofascial e com várias formas de distúrbios internos da ATM. Vários mecanismos inflamatórios e degenerativos agem em conjunto para produzir os sinais e sintomas e características fisiopatológicas da Osteoartrite da ATM.
