Resumo: | Introduction: There are few reports about the impact of SARS-CoV-2 virus infection on children under 5 years of age in Brazil´s northeastern is no well-known. Objectives: To evaluate the clinical, demographic, laboratory characteristics and outcomes of children under five years of age with COVID-19, observed from April 2020 to April 2021 in the city of Cajazeiras, Paraíba. Material and Method: An observational, analytical, retrospective cohort study was conducted, using data collected from municipal epidemiological surveillance (E-SUS VE, SIVEP-flu), hospital records and home visits, of COVID-19 cases in children under 5 years of age, laboratory confirmed, in the first year of the pandemic, in the municipality of Cajazeiras, Paraíba. The database was created using SPSS software, and Pearson's chi-square or Fisher's exact tests were used for inferential statistical techniques, with a margin of error of 5.0% and intervals with a reliability of 95.0%. Results: Bivariate analysis showed a statistically significant association between age groups and clinical characteristics such as fever; coryza; headache; taste disorders; hypoactivity and elevated C-reactive protein. As well as, between the outcomes and the presence of history of: neuropathy; lung disease; bronchopneumonia; diabetes and neonatal O2 use. The most frequent diagnoses were: pneumonia; pleural effusion; ITRI; NRTI and Streptococcus Septicemia. In 18.9% of cases, clinical manifestations were observed after the acute phase of Covid-19. Conclusion: The reduced number of hospitalizations (2.2%) and the absence of deaths may suggest possible underdiagnosis and underreporting of deaths. Active surveillance and differentiated care are recommended for children with chronic diseases, who have vulnerability factors for the aggravation of COVID-19. Por conseguinte, torna-se relevante a geração dessas evidências, dada a singularidade que permeia o espaço clínico inerente à faixa etária pediátrica. Objetivo: Avaliar as características clínicas, demográficas, laboratoriais e os desfechos das crianças menores de cinco anos com COVID-19, observadas no período de abril de 2020 a abril de 2021 na cidade de Cajazeiras, Paraíba. Material e Método: Foi conduzido estudo observacional, analítico, de coorte retrospectiva, utilizando dados coletados na vigilância epidemiológica municipal (e-SUS Vigilância Epidemiológica, Sistema de Informação de Vigilância da Gripe), em prontuários hospitalares e visitas domiciliares, dos casos de COVID-19 em crianças menores de cinco anos, confirmados laboratorialmente, no primeiro ano da pandemia. Os dados foram analisados por meio da estatística descritiva e inferencial, adotados os testes χ2 de Pearson ou Exato de Fisher, com margem de erro de 5,0% e intervalos de confiança (IC) de 95,0%. Resultados: Considerando os 4.984 casos de COVID-19 confirmados entre todas as faixas etárias, 3,61% foram constituídos por população de crianças menores de cinco anos. Cento e oitenta casos confirmados laboratorialmente foram incluídos no estudo, sendo 53% do sexo feminino, média de idade de 2,19 anos, mediana de 2,00 e desvio padrão de 1,7. A maioria dos casos (27,2%) ocorreu na faixa etária de 48–50meses e 29 dias, e a menor ocorrência (10,6%), entre 6–11 meses e 29 dias. A análise bivariada mostrou associação estatisticamente significante entre faixas etárias e características clínicas como febre, coriza, cefaleia, distúrbios gustativos, hipoatividade e proteína C reativa elevada, bem como entre os desfechos e a presença de antecedentes de: neuropatia, pneumopatia, broncopneumonia, diabetes e uso de oxigênio neonatal. Os diagnósticos mais frequentes foram: pneumonia, derrame pleural, infecção do trato respiratório inferior, infecção do trato respiratório superior e septicemia por estreptococos. Em 18,9% dos casos foram observadas manifestações clínicas após a fase aguda da COVID-19. Conclusão: O reduzido registro de hospitalizações (2,2%) e a inexistência de óbitos podem sugerir possíveis fragilidades relacionadas a ocorrências de subdiagnóstico e subenumeração de óbitos. Recomendam-se vigilância ativa e cuidados diferenciados às crianças portadoras de doenças crônicas, as quais apresentaram fatores de vulnerabilidade para agravamento da COVID-19.Palavras-chave: Criança, SARS-CoV-2, COVID-19, Coronavírus, Infecção. ABSTRACT Introduction: The understanding of morbidity and mortality in children infected by SARS-CoV-2 presents scarce evidence in the Northeast region of Brazil. Therefore, to provide this evidence becomes relevant, given the uniqueness that permeates the clinical space inherent to the pediatric age group. Objective: To evaluate the clinical, demographic, laboratory characteristics and outcomes of children under five years of age with COVID-19, observed from April 2020 to April 2021 in the city of Cajazeiras, Paraíba. Method: An observational, analytical, retrospective cohort study was conducted, using data collected from the municipal epidemiological surveillance (Electronic Unified Health System Epidemiological Surveillance Notification Registration System — E-SUS VE, Influenza Surveillance Information System — SIVEP-influenza), hospital records and home visits of laboratory-confirmed COVID-19 cases in children under five years of age in the first year of the pandemic. Data were analyzed using descriptive and inferential statistics, using Pearson’s χ2 test or Fisher’s exact test, with a margin of error of 5.0% and confidence intervals (CI) of 95.0%. Results: Considering the 4,984 confirmed cases of COVID-19 among all age groups, 3.61% were made up of children under five. A hundred and eighty laboratory-confirmed cases were included in the study, 53% female, mean age of 2.19 years, median 2.00, and standard deviation of 1.7. Most cases (27.2%) occurred in the age group of 48–50 months and 29 days, and the lowest occurrence (10.6%) was between 6–11 months and 29 days. The bivariate analysis showed a statistically significant association between age groups and clinical characteristics such as fever; runny nose; headache; taste disorders; underactivity; and elevated C-reactive protein. This was also the case between the outcomes and the presence of antecedents such as: neuropathy; lung disease; bronchopneumonia; diabetes; and neonatal O2 use. The most frequentdiagnoses were pneumonia; pleural effusion; lower respiratory tract infection; upper respiratory tract infection and Streptococcus Septicemia. In 18.9% of cases, clinical manifestations were observed after the acute phase of COVID-19. Conclusion: The reduced record of hospitalizations (2.2%) and absence of deaths may suggest possible weaknesses related to occurrences of underdiagnosis and underreporting of deaths.
