Resumo: This paper discusses the decentralization process associated with the State reform, with special emphasis to the health sector, covering a wide range of problems related to the implementation of the Unified Healthcare System (SUS). Two aspects serve as analytical guides for the evaluation of health policy decentralization: on one hand, the decentralization of funds according to the country's regions and their repassage to the private and philanthropic sector vis-à-vis the direct transfers to states and municipalities; on the other, health policy regulation by means of the basic operational rules, as well as the procedures that entitle the municipalities to the decentralized management model.
