Pacientes de alta que permanecem dentro do hospital: quem são e por que permanecem?

The existence of specific hospitals for long stay patients is justified by the need for specialized professionals and care. The process leading to hospital discharge in these institutions has been more complex than just health condition, including factors that delay discharge of the patient. This study aimed to identify reasons why patients remain hospitalized in a long-stay public hospital located in the Alto Tietê region, Greater São Paulo. A cross-sectional descriptive exploratory study identified 1,211 hospital admissions between January 2011 and December 2014 with a mean age of 53.9 years and male predominance (62.7%). From the 822 admissions analyzed, delayed hospital discharge occurred in 466 cases (56.7%). The average delay was 19.1 days (range 1-606 days) and 8,895 inpatient days which could be avoided. The main causes of delay discharge were family transportation (39.7%), awaiting ambulance (14.8%), waiting bed in other place (12.7%), family resistance (12.4%) and waiting medical equipment/house adaptation (9.4 %). There are several ways proposed to reduce the length of stay because the causes of delayed discharge result from a wide range of contributing factors. However, measures that should be taken are based on a proper understanding of the local contexts (health system, cultural and legal).
