Perception of managers, providers and auditors of contracting in the Brazilian Unified Health System

Resumo: The objective of the study was to understand the perception of managers, providers and auditors of the process of contracting within the Brazilian Unified Health System (Sistema Único de Saúde). This was a descriptive study, using a qualitative approach, carried out in Maringá, Paraná, in September and October 2017, with managers, service providers and auditors who work in the system. Data were collected through guiding questions. The analysis led to the definition of two categories: opportunities and barriers in the contracting process with the Health System; evaluation and use of instruments in the System audit. Regarding the opportunities, it was evidenced that the contracting process improves the quality of health management based on specific legislation and that the main barrier is related to the scale of values practiced by the Health System. In relation to the second category, the analysis indicates that the use of instruments allows the standardization of parameters, which provides greater assertiveness to the contracting process. We conclude that it is necessary to use an audited instrument validated and agreed with providers, in order to ensure transparency and better results in term of the agreed upon goals.

Este fue un estudio descriptivo, con un enfoque cualitativo, realizado en Maringá, Paraná, en septiembre y octubre de 2017, con gestores, proveedores de servicios y auditores que actúan en el Sistema. Los datos se recopilaron por medio de preguntas orientadas. El análisis llevó a la construcción de dos categorías: potencialidades y barreras en el proceso de contratación con el Sistema Único de Salud; evaluación y uso de instrumentos en la auditoría del Sistema. Con respecto a las potencialidades, se constató que el proceso de contratación califica la gestión de la salud basada en las legislaciones específica y que la barrera principal se refiere a la tabla de valores practicadas por el Sistema Único de Salud. Con respecto a la segunda categoría, se analizó que el uso de instrumentos permite la estandarización de parámetros, lo que le da al contrato una mayor asertividad. Concluimos que es necesario utilizar un instrumento de auditoria validado y en acuerdo con los proveedores, para tener transparencia y mejores resultados en los objetivos acordados.O objetivo do estudo foi compreender a percepção de gestores, prestadores e auditores sobre o processo de contratualização do Sistema Único de Saúde. Tratou-se de um estudo descritivo, com abordagem qualitativa, realizado em Maringá, Paraná, em setembro e outubro de 2017, com gestores, prestadores de serviços e auditores que atuam no Sistema. Os dados foram coletados por meio de questões norteadoras. A análise levou à construção de duas categorias: potencialidades e barreiras no processo de contratualização com o Sistema Único de Saúde; avaliação e uso de instrumentos na auditoria do Sistema. Em relação às potencialidades, evidenciou-se que o processo de contratualização qualifica a gestão da saúde baseada nas legislações específicas e que a principal barreira se refere à tabela de valores praticada pelo Sistema Único de Saúde. Em relação à segunda categoria, analisou-se que o uso de instrumentos permite a padronização de parâmetros, o que confere à contratualização maior assertividade.
