Scope of actions of community health workers in the COVID-19 pandemic: literature review

Resumo: The objective of this article was to systematize scientific evidence on the work of community health workers during the pandemic period between 2020 and 2022 in countries in Africa, Asia, Europe and North America. The methodology used was literature review and evidence synthesis in the PubMed and Web of Science databases referring to international scientific productions on the community health workers’ actions. A total of 23 studies were selected for analysis in the categories: characterization of studies; scope of routine actions; and changes in the work process during the pandemic. The scope of the program of these professionals was presented in different ways in the countries studied. Even with interruptions or adaptations, some routine actions were maintained. However, its attributions suffered considerable changes due to the need for social distancing, especially the use of technologies. Strong evidence of its relevance was identified in countries that had or did not have the community health workers program previously. Thus, in view of the comprehensive scope of action, the community health workers should not constitute a temporary solution and not integrated with the formal health organization, but as a fundamental part of a health system to achieve the improvement of the population quality of life.

La metodología utilizada fue la revisión de la literatura y la síntesis de la evidencia en las bases de datos de PubMed y en la Web of Science relativa a las producciones científicas internacionales sobre las acciones de los agentes comunitarios de salud. Se seleccionaron 23 estudios para su análisis en las categorías: caracterización de los estudios; alcance de las acciones rutinarias; y cambios en el proceso de trabajo durante la pandemia. El alcance del programa de estos profesionales se presentó de diferentes maneras en los países estudiados. Incluso con las interrupciones o adaptaciones, se han mantenido algunas acciones de rutina. No obstante, sus asignaciones han sufrido cambios considerables debido a la necesidad de distanciamiento social, destacando el uso de tecnologías. Se ha identificado una fuerte evidencia de su relevancia en países que antes tenían o no el programa de agentes comunitarios de salud. Por lo tanto, frente al amplio alcance de las actividades, los agentes de salud comunitarios no deberían constituirse en una solución temporal y poco integrada para la organización formal de la salud, sino como parte fundamental de un sistema de salud que permita mejorar la calidad de vida de la población.O objetivo deste artigo foi sistematizar evidências científicas sobre o trabalho dos agentes comunitários de saúde durante o período pandêmico entre 2020 e 2022 em países da África, Ásia, Europa e América do Norte. A metodologia utilizada foi revisão de literatura e síntese de evidências nas bases de dados PubMed e Web of Science referentes às produções científicas internacionais sobre as ações dos agentes comunitários de saúde. Foram selecionados 23 estudos para análise nas categorias: caracterização dos estudos; escopo das ações rotineiras; e alterações no processo de trabalho durante a pandemia. O escopo do programa desses profissionais se apresentou de diferentes formas nos países estudados. Mesmo com interrupções ou adaptações, mantiveram-se algumas ações de rotina. No entanto, suas atribuições sofreram mudanças consideráveis pela necessidade do distanciamento social, destacando-se o uso de tecnologias. Identificaram-se fortes evidências da sua relevância em países que possuíam ou não o programa de agentes comunitários de saúde anteriormente.
