Building a "Smiling Brazil"? Implementation of the Brazilian National Oral Health Policy in a health region in the State of São Paulo

Resumo: This paper is a case study on the implementation of the Brazilian National Oral Health Policy (PNSB), known as "Smiling Brazil", in the cities of the Regional Health Department of Araraquara (DRS III) in São Paulo State. A structured questionnaire was given to the municipal oral health coordinators, an interview with oral health care professionals and managers was conducted, and the official data provided by the Brazilian Ministry of Health were coded to assess the policy's scope: (i) expansion and qualification of actions; (ii) work conditions; (iii) care; (iv) access; and (v) planning and management. The quantitative and qualitative analyses were linked by methods triangulation. In terms of PNSB implementation, the majority of the cities (52.6%) were classified as "good", with 42.1% classified as "bad". Approximately 10 years after launching the PNSB, despite strides in oral health care and access to different levels of care, the cities still experience difficulties in implementing the policy's principles.

Se aplicó un cuestionario a los coordinadores municipales de salud bucal, una entrevista con directores y profesionales de salud bucal, y las estadísticas oficiales del Ministerio de Salud para evaluar las dimensiones de la PNSB: (i) Ampliación y calificación de las acciones; (ii) Condiciones de trabajo; (iii) Atención; (iv) Acceso; y (v) Planificación y Gestión. La articulación de las evaluaciones cuantitativa y cualitativa se llevó a cabo desde la perspectiva de triangulación de métodos. La mayoría (52,6%) de las ciudades se ubicó en la clasificación "buena" y un 42,1% obtuvo clasificación "mala" en la ejecución de la PNSB. Pasados 10 años del lanzamiento de la PNSB, a pesar de que se han observado avances en el cuidado de la salud bucal y en el acceso a los diferentes servicios de los niveles de atención, todavía se presentan dificultades para poner en práctica sus directricesEste artigo é um estudo de caso do processo de implementação da Política Nacional de Saúde Bucal (PNSB) - "Brasil Sorridente" - nos municípios do Departamento Regional de Saúde de Araraquara (DRS III), São Paulo, Brasil. Foram realizadas aplicação de questionário estruturado aos coordenadores municipais de saúde bucal, entrevistas com gestores e profissionais de saúde bucal, observação dos serviços e sistematização de estatísticas oficiais do Ministério da Saúde, no sentido de avaliar as dimensões da PNSB: (i) Ampliação e qualificação das ações; (ii) Condições de trabalho; (iii) Cuidado; (iv) Acesso; (v) Planejamento e Gestão. A articulação das análises quantitiva e qualitativa foi realizada na perspectiva da triangulação de métodos. A maioria (52,6%) dos municípios obteve classificação "boa" e 42,1% estão em uma situação "ruim" de implementação da PNSB.
