IMPORTANCE OF ECTOSCOPIC EXAMINATION IN THE EARLY DETECTION OF MELANOMA

Resumo: | Introduction: In Brazil, skin cancer is responsible for one third of malignant neoplasms, and melanoma is related to high mortality, depending on the stage at diagnosis. Objective: To report a case in which two lesions suggestive of ectoscopic malignancy were identified, in a consultation to which the patient did not refer such injuries. We demonstrate the importance of a rigorous dermatological examination in order to identify potentially malignant lesions and promote a better quality of life for the patient, regardless of the patient's main complaint. Case Description: A 69-year-old patient attended the dermatological consultation complaining of an unsatisfactory aesthetic procedure. The dermatological examination identified pigmented, asymmetric lesions, with irregular edges and varied colors. The histopathological result indicated lentigo malignant melanoma "in situ". The patient had been referred for surgical treatment, under guidance. Discussion: It is of great importance that the doctor and the general population know the ABCDE rule for skin cancer, in order to pay attention to dermatological lesions with suspected malignancy, since patients with melanomas detected early, either by clinical examination, or be assisted by dermoscopy, have better survival, lower overall mortality and less cancer-related morbidity. Conclusion: It is necessary to emphasize the need for the knowledge of suspected pigmented lesions by non-specialist doctors, as well as the population's awareness of this identification and its consequences. |

Introdução: No Brasil, o câncer de pele é responsável por um terço das neoplasias malignas, sendo o melanoma relacionado à alta mortalidade, dependendo de seu estágio ao diagnóstico. Objetivo: Relatar um caso em que foram identificadas duas lesões sugestivas de malignidade à ectoscopia, em uma consulta a qual paciente não se referia à tais lesões. Demonstramos a importância do exame dermatológico minusioso e completo, afim de identificar lesões potencialmente malignas e promover uma melhor qualidade de vida ao paciente, independente da queixa principal do mesmo. Relato de caso: Paciente de 69 anos compareceu a consulta dermatológicas com queixas acerca de um procedimento estético insatisfatório. Ao exame dermatológico, foram identificadoas lesões pigmentadas, assimétricas, de bordas irregulares e cores variadas. O resultado histopatológico indicou lentigo maligno melanoma “in situ”. Paciente fora encaminhada para tratamento cirúrgico, sob orientações. Discussão: É de grande importância que tanto os médicos quanto a população em geral tenha conhecimento da regra ABCDE para câncer de pele, visando atentar para lesões dermatológicas suspeitas de malignidade, uma vez que os pacientes com melanomas detectados precocemente, tanto através do exame clínico, como auxiliado pela dermatoscopia, têm melhor sobrevida, menor mortalidade geral e menor morbidade relacionada ao câncer.
