Primary progressive aphasialogopenic type

Resumo: | Primary Progressive Aphasia (PPA) is an insidious neurodegenerative syndrome, which mainly affects language, whose deterioration is progressive. The initial presentation is the difficulty of appointment, as in the early stages of Alzheimer’s disease (AD). Objectives: Report a case of APP in logopenicform, conduct a review of the specialized literature. Discuss the importance of correct diagnosis and differential diagnosis. Case Report: 74 yersold woman, previously healthy, maintaining their daily activities started having difficulty to recognize and name the household objects. Family report worsening in recent memory in this period, with episodes associated confusion. A year after the start of the frame to the patient could not manage more daily activities, requiring assistance for activities which initially played alone. During this period he was diagnosed with AD. Clinical presentation worsened over the months, with worsening of interaction, appointment, little fluent speech, repeating with difficulty what is asked to speak. MRI shows atrophy in frontotemporal region left. Conclusions:Logopenic form of APP, patient aspect in question, manifests itself with some fluent speech, with a slow pace and with long pauses anomic, as the beginning of the frame. The repetition and understanding to more complex grammatical structure of sentences are disturbed, but preserved for single words. There may be limited and highly expressed difficulty in understanding the word, which was observed in this patient after two years of clinical evolution. The main differential diagnosis is AD. |

The main differential diagnosis is AD.A Afasia Progressiva Primária (APP) é uma síndrome neurodegenerativa de instalação insidiosa, que afeta principalmente a linguagem, cuja deterioração é progressiva. A apresentação inicial é a dificuldade de nomeação, como ocorre na fase inicial da Doença de Alzheimer (DA). Objetivos:Relatar um caso de APP na sua forma logopênica, realizar uma revisão da literatura médica especializada. Discutir a importância do correto diagnóstico e diagnósticos diferenciais. Relato do Caso:Mulher,74 anos, previamente hígida, mantendo suas atividades diárias, iniciou quadro com dificuldade de reconhecer e nomear os objetos de casa. Familiares relatam piora da memória recente nesse período, com episódios de confusão associada. Um ano após o inicio do quadro a paciente não conseguia gerir mais suas atividades diárias, necessitando de auxilio para as atividades que inicialmente desempenhava sozinha. Nesse período foi feito diagnóstico de DA. O quadro piorou com o passar dos meses, com piora da interação, da nomeação, discurso pouco fluente, repetindo com dificuldade o que se pede para falar. RNM demonstra atrofia em região fronto-temporal à esquerda. Conclusões: A forma logopênica da APP, aspecto da paciente em questão, se manifesta com um discurso pouco fluente, com um ritmo lento e com pausas anômicas longas, como foi o inicio do quadro. A repetição e a compreensão para frases de estrutura gramatical mais complexa estão perturbadas, mas preservadas para palavras isoladas. Pode haver expressão limitada e grande dificuldade na compreensão da palavra, fato observado nessa paciente após dois anos de evolução clínica.
