Non-Hodgkin’s lymphoma of mantle cell: Case Report

Resumo: Introduction: Lymphomas are one group of malignancies originating in lymphoid tissue and the mantle cell lymphoma is a subtype of non-Hodgkin’s lymphoma (NHL) derived from small “virgin” B lymphocytes of the mantle zone. It corresponds to 6% of all NHL. Objective: To describe a case of non-Hodgkin´s lymphoma mantle cell. Method: Review of medical records. Case Report: J.N.A, 62 years old, male, with a history of painless left cervical lymphadenopathy, followed by the appearance of right and left axillary, bilateral inguinal and supraclavicular limphadenopathies, and painless enlargement of the scrotum. He reported night sweats, followed by asthenia and weight loss. On examination: firm lymph nodes adhered to deep planes, elastic consistency. Abdomen: distended, depressing, painless to superficial and deep palpation, presenting hepatosplenomegaly. Chest radiograph presenting bilateral mediastinal lymphadenopathy. The diagnoses was made by cervical lymph node biopsy: non-Hodgkin’s lymphoma of mantlecell, showing immunohistochemistry with positive CD20 and Cyclin D1. Iliac bone marrow biopsy on the right and left: diffuse infiltration by non-Hodgkin’s small cells, consistent with mantle cell lymphoma, representing 90% of the total cellularity. It was accomplished steroid pulse therapy and after, initiated chemotherapy with CHOP scheme. At this moment, he is in multidisciplinary follow. Conclusion: This case portrays an aggressive lymphoma with disseminated disease (stage IV of Ann Harbor) and prognostic index (IPI) with high scores indicating a worse prognosis. In these cases the treatments have been unsatisfactory, with complete remission achieved by a minority, with median survival of 3 to 5 years.

Corresponde a 6% de todos os LNH.Objetivo: Descrever um caso de linfoma não-Hodgkin de células do manto.Método: Revisão de prontuário médico.Relato de Caso: J.N.A, 62 anos, sexo masculino, com história de adenomegalias cervicais indolores à esquerda, seguidas pelo aparecimento de adenomegalias axilares à direita e esquerda; inguinais bilaterais; supra-claviculares e aumento indolor da bolsa escrotal. Relata sudorese noturna, acompanhada de astenia e emagrecimento. Ao exame: Linfonodos firmes, aderidos a planos profundos, de consistência elástica. Abdômen: globoso, depressível, indolor à palpação superficial e profunda, apresentando hepatoesplenomegalia. Radiografia de tórax apresentando adenomegalia mediastinal bilateral. O diagnóstico foi realizado por biopsia do linfonodo cervical: linfoma não-Hodgkin de células do manto, apresentando imunohistoquimica com CD20 e Ciclina D1 positivos. Biopsia da medula óssea ilíaca direita e à esquerda: Infiltração difusa por Linfoma não-Hodgkin de pequenas células, compatível com linfoma de células do manto,correspondendo a 90% da celularidade total. Feito pulsoterapia com corticóide e após, inicio da quimioterapia com esquema CHOP. No momento em acompanhamento multidisciplinar.Conclusão: Este caso retrata um linfoma agressivo, com doença disseminada (estágio IV de Ann Harbor) e índice de prognóstico (IPI) com escore elevado, indicando um pior prognóstico.
