The purpose of this review is to highlight the importance of the treatment of hypothyroidism in pregnant women, in order to avoid maternal and neonatal complications, such as premature labor, intrauterine growth restriction, fetal neurocognitive impairment and placental abruption. The main cause of hypothyroidism in gestation is chronic autoimmune thyroiditis, known as Hashimoto's thyroiditis. Other causes are therapies performed with previously radioiodine and thyroidectomy. Pituitary hypothyroidism is rare during pregnancy. In endemic areas, iodine deficiency affects about 1.2 billion people. Hypothyroidism is defined by the increase in TSH levels and the reduction of free T4. In groups of women at higher risk for developing this disease, the dosage of HRT and free T4 should be performed at the first prenatal visit, aiming at maternal and fetal well-being. Treatment should be carried out during gestation with special considerations, as they can cause harm to the fetus and the mother.
