Echocardiographic diagnostic findings in the setting of pulmonary thromboembolism: relato de caso

Resumo: Pulmonary thromboembolism (PTE) has high mortality if not promptly treated. This case report describes a 67-year-old male patient that experienced syncope and elevated markers of myocardial necrosis. Coronary angiography showed no obstructive lesions. A transthoracic echocardiogram revealed an enlarged right ventricular diameter, borderline right ventricular function, the presence of McConell's sign, and an increased pulmonary artery systolic pressure (PASP). Combined with other diagnostic tests, the diagnosis of pulmonary thromboembolism (PTE) was confirmed. After three months of anticoagulation therapy, there was an improvement in right ventricular function indices and normalization of PASP. This case report has the following learning objectives: 1. Recognize the significant correlation between syncope and PTE as a clinical manifestation. 2. Evaluate and demonstrate the diagnostic role of echocardiography in PTE. 3. Identify and illustrate key echocardiographic alterations associated to this condition. 4. Discuss the implications of echocardiography in the management and treatment decisions for patients with PTE.

A partir de tal constatação, este relato de caso visa descrever um paciente do sexo masculino, de 67 anos, que apresentou um episódio de síncope e elevação de marcadores de necrose miocárdica. A angiografia coronária não revelou lesões obstrutivas. O ecocardiograma transtorácico revelou aumento do diâmetro do ventrículo direito, função ventricular direita limítrofe, presença do sinal de McConell e aumento da pressão sistólica da artéria pulmonar (PSAP). Combinado com outros exames complementares, o diagnóstico de tromboembolismo pulmonar (TEP) pôde ser confirmado. Após três meses de anticoagulação, houve melhora dos índices de função ventricular direita e normalização da PSAP. Este relato de caso tem os seguintes objetivos de aprendizagem: 1. Reconhecer a correlação significativa entre TEP e síncope como manifestação clínica. 2. Avaliar e demonstrar o papel diagnóstico da ecocardiografia no TEP. 3. Identificar e ilustrar as principais alterações ecocardiográficas associadas a essa doença. 4.
