The use of hydrochlorothiazide and erectile dysfunction

Resumo: Erectile dysfunction (ED) is defined as the inability to achieve or maintain the penis with an erection sufficient for sexual intercourse to occur. The fact that the development of ED is multifactorial makes it difficult to identify the cause of the problem, but the use of medications routinely used to treat hypertension, such as thiazide diuretics, may be possibly responsible for the development of ED, especially among hypertensive patients. This is a descriptive study based on a bibliographic review, establishing an active search in the PubMed and Google Acadêmico databases for the terms ‘hydrochlorothiazide’ and ‘erectile dysfunction’. The objective of this study is to identify possible mechanisms that explain the effect of using hydrochlorothiazide on the emergence of erectile dysfunction. The hypotheses of this study establish that hydrochlorothiazide may be related to the development of erectile dysfunction due to arterial calcification, reduction in testosterone levels and increased contractility of the corpus cavernosum resulting from changes in intracellular calcium levels. Furthermore, the decrease in zinc levels resulting from the use of the medication can promote oxidative stress, leading to a reduction in testosterone and the availability of nitric oxide, essential for the erection process.

O fato do desenvolvimento da DE ser multifatorial dificulta a identificação da causa do problema, mas o uso de medicamentos utilizados rotineiramente para o tratamento da hipertensão, como os diuréticos tiazídicos, pode ser um possível responsável pelo desenvolvimento da DE sobretudo entre os hipertensos. Este é um estudo descritivo baseado em revisão bibliográfica, estabelecendo uma busca ativa no banco de dados PubMed, Scielo e Google Acadêmicopelos termos ‘hydrochlorothiazide’ e ‘erectile dysfunction’. O objetivo deste estudo é levantar possíveis mecanismos que expliquem o efeito do uso da hidroclorotiazida no surgimento da disfunção erétil. As hipóteses estabelecem que a hidroclorotiazida pode estar relacionada ao desenvolvimento da disfunção erétil pela calcificação arterial, redução nos níveis de testosterona e aumento da contratilidade do corpo cavernoso resultantes da alteração dos níveis intracelulares de cálcio.
