Epidemiological profile of children and adolescents diagnosed with tuberculosis at the Reference Center Augusto Guimarães in the period 2008-2012

Resumo: Tuberculosis is a disease with high incidence in Brazil, recognized, particularly in Campos (RJ) as a public health problem. Epidemiological evidence of tuberculosis in children and adolescents are scarce generating diagnostic difficulties. The disease in children is paucibacilífera and obtaining sputum is difficult. Tuberculosis in adolescents is didactically compared with the disease in children, but forms of disease similar to those found in adults with active tuberculosis are found. Objective: To investigate the epidemiology of tuberculosis in childhood and adolescence, in Campos (RJ). Methods: The study examined the type of documentary registration sheets of 108 children and adolescents, 0-19 years diagnosed with tuberculosis at the Reference Center Augusto Guimarães, in the period 2008-2012. Variables: patients with tuberculosis, age, sex, BAAR, PPD, chest radiography, clinical form, number of contacts and the input type. The data were taken from the log book provided by the health unit, statistically analyzed and presented through graphs. This study met the Resolution nº 196/ 1996 of the CNS on the research with human being adopted with the opinion paragraph 189 817 and 253 128. Results: Of 108 cases of tuberculosis, 73 % were adolescents (10- 19 years). Males predominated with 63 %, 54% of adolescents had positive BAAR and 76 % of children showed up reactive to PPD. The suggestive radiographic pattern prevailed in 94% of cases and how pulmonary clinic with 83%. In 24% of cases the contacts were ignored and 93 % came as a new case. Conclusion: The epidemiological profile was found among adolescents 10 to 19 years, were predominantly male, pulmonary form and suggestive pattern on chest radiography. The active search for contacts should be seen as an important preventive measure in tuberculosis control; encourages the attendance of patients to the clinic for examination and treatment, favors the diagnosis in the initial phase and interferes in reducing the incidence and mortality of children and adolescents tuberculosis. Os dados epidemiológicos da tuberculose na criança e no adolescente são escassos gerando dificuldades diagnósticas. A doença nas crianças é paucibacilífera e, a obtenção do escarro é difícil. A tuberculose em adolescentes é comparada didaticamente com a doença nas crianças, mas são encontradas formas da doença semelhantes às encontradas em adultos bacilíferos. Objetivo: Verificar o perfil epidemiológico da tuberculose na infância e na adolescência, em Campos (RJ). Métodos: Estudo do tipo documental analisou as folhas de registro de 108 crianças e adolescentes, de 0 a 19 anos, diagnosticadas com tuberculose no Centro de Referência Augusto Guimarães, no período de 2008 a 2012. Variáveis: portadores de tuberculose, idade, sexo, BAAR, PPD, radiografia de tórax, forma clínica, número de contatos e tipo de entrada. Os dados foram retirados do livro de registro disponibilizado pela unidade de saúde, analisados estatisticamente e apresentados através de gráficos. Este estudo atendeu a Resolução nº 196/ 1996 do CNS relativa à pesquisa com humanos sendo aprovada com o parecer nº 189.817 e 253.128. Resultados: Dos 108 casos de tuberculose, 73% eram adolescentes (10 a 19 anos). Predominou o sexo masculino com 63%; 54% dos adolescentes apresentaram BAAR positivo e 76% das crianças mostraram-se reatoras ao PPD. O padrão radiográfico sugestivo predominou em 94% dos casos e a forma clínica pulmonar com 83%. Em 24% dos casos os contactantes foram ignorados e 93% apresentaram-se como caso novo. Conclusão: O perfil epidemiológico encontrado foi de adolescentes entre 10 a 19 anos; houve predominância do sexo masculino, forma clínica pulmonar e padrão sugestivo na radiografia de tórax. A busca ativa por contactantes deve ser encarada como medida preventiva importante no controle da tuberculose; incentiva o comparecimento dos doentes à unidade de saúde para exame e tratamento, favorece o diagnóstico em fase inicial e interfere na redução da incidência e morbimortalidade de crianças e adolescentes por tuberculose.
