Emphysematous pyelonephritis in a patient with a solitary kidney: a case report

Resumo: The emphysematous pyelonephritis (EP) is a rare disorder characterized by severe necrotizing infection and potentially fatal kidney parenchyma, in which one observes the formation of gas in its interior as well as the collection system and/or tissue near kidney. It is evident striking relationship with diabetes mellitus and obstructive uropathy, with significant mortality. Objective: To report a case of emphysematous pyelonephritis in a patient with solitary kidney. Descriptions: Female patient, 26, referring back pain right at high intensity, followed by vomiting and poor general condition. Computerized tomography of the abdomen demonstrated calculations the right kidney and intraparenchymal gas, suggesting emphysematous pyelonephritis. She was treated conservatively and obtained favorable clinical evolution. Conclusions: PE is an acute infection characterized by the presence of air in the renal parenchyma and surrounding tissues, where the main etiologic agent is E. coli. The abdominal CT is the method of choice for diagnosis. Treatment depends on the CT findings. The patient in this report, classified as class IV according to the scale of Huang and Tseng, obtained favorable clinical evolution with only conservative treatment, not requiring surgical intervention, in contrast to the data obtained in the literature. It also contrasts the fact that the patient be young, not having fever during the course of the disease and negative urine culture.

It also contrasts the fact that the patient be young, not having fever during the course of the disease and negative urine culture.A pielonefrite enfisematosa (PE) é uma entidade clínica rara, caracterizada pela infecção necrotizante grave e potencialmente fatal do parênquima renal, no qual se observa a formação de gás em seu interior, bem como no sistema coletor e/ou tecido perinéfrico. Evidencia-se notória relação com o diabetes mellitus e com a uropatia obstrutiva, apresentando mortalidade significativa. Objetivo: Relatar um caso de pielonefrite enfisematosa em paciente com rim único. Descrições: Paciente do sexo feminino, 26 anos, referindo lombalgia à direita, de forte intensidade, acompanhada de vômitos e queda do estado geral. Realizou tomografia computadorizada de abdome, onde foram evidenciados cálculos em rim direito, além de gás intraparenquimatoso, sugerindo um quadro de pielonefrite enfisematosa. Foi tratada conservadoramente e obteve evolução clínica favorável. Conclusões: A PE é uma infecção aguda caracterizada por presença de ar no parênquima renal e tecidos circunvizinhos, onde o principal agente etiológico é E. coli. A TC de abdome é o exame de eleição para o diagnóstico. O tratamento depende dos achados tomográficos. A paciente do presente relato, classificada como classe IV segundo a escala de Huang e Tseng, obteve evolução favorável do quadro clínico apenas com tratamento conservador, não necessitando de intervenção cirúrgica, contrastando com os dados levantados na literatura.
