Green pigmentation of primary dentition associated with hyperbilirrubinemia at neonatal period

Resumo: | Introduction: Green pigmentation of teeth is rare but it causesstrong anxiety for child and parents. It results from a teeth reaction indevelopment to a blood supply rich in bilirrubin. Magnitude andextension of pigmentation varies with grade of seric bilirrubin andstage of odontogenesis that is confirmed by selective involvement ofteeth and absence of pigmentation of definitive dentition.Objective: To report a case of one toddler with green staining ofdeciduous teeth.Method: Review of medical and odontological files.Case report: A two year-old female child, Caucasian, was admittedat a pediatric odontological service for evaluation of greenpigmentation of teeth. At physical examination it was observed greenstaining of all deciduous teeth, most severe in upper and lowerincisors and distributed with variation in degree and extensionaccording chronology of odontogenesis. Soft tissues were pre-served and with normal staining. The child was premature evolvingwith sepsis, severe hiperbilirrubinemia, policitemia and prolongedremaining at neonatal intensive care unit.Conclusion: Green pigmentation of dentition is an uncommon entitydemanding individualized treatment and knowledge of etiology thatin most of relates of cases serious episodes of hiperbilirrubinemia atneonatal period caused by sepsis, blood incompatibility or biliaryatresia. Policitemia as possible adjuvant factor is at first time related. |

Ela resulta deuma reação dos dentes em desenvolvimento, ao suprimentosanguíneo rico em bilirrubina. A intensidade e a extensão dapigmentação variam com os níveis de bilirrubina sérica e a fase daodontogênese, o que é comprovado pelo acometimento seletivo dealguns dentes e pela ausência de pigmentação nos dentes definitivos.Objetivo: Relatar um caso de pré-escolar portadora de dentesdecíduos com pigmentação verde.Método: Revisão de prontuários médico e odontológico.Relato de caso: Criança do sexo feminino, dois anos de idade,leucoderma, admitida em serviço de Odontopediatria para avaliaçãoda coloração verde de seus dentes. Ao exame clínico, observou-secoloração verde em todos os elementos dentários decíduos, sendoesta mais acentuada nos incisivos superiores e inferiores, estandodistribuída com variação em grau de acometimento e extensão, deacordo com a cronologia da odontogênese. Os tecidos molesapresentavam-se íntegros e com coloração normal. A criança nasceuprematura evoluindo com sepsis, severa hiperbilirrubinemia,policitemia e permanência prolongada em unidade de terapiaintensiva neonatal.Conclusão: A pigmentação verde dos dentes é uma entidadeincomum exigindo conduta terapêutica individualizada e oreconhecimento da etiologia, que na maior parte dos casos remontaa severos episódios de hiperbilirrubinemia no período neonatalcausados por sepsis, incompatibilidade sanguínea ou atresia devias biliares.
