ACOMPANHAMENTO FARMACOTERAPÊUTICO DE PACIENTES IDOSOS RESIDENTES DE UMA ENTIDADE DE LONGA PERMANÊNCIA

Resumo: Pharmacotherapeutic follow-up is a practice within pharmaceutical care, which consists of investigating the dosage, means of access, adherence and administration of the medication, as well as the study of adverse reactions and drug interactions of the therapy used by the patient. The elderly population is the one that has the most diseases, therefore it is the one that most uses health services. It is estimated that the more medications the elderly person uses, the greater the risk of having a drug interaction. The objective of the present study was to carry out pharmacotherapeutic follow-up of the elderly, aiming at improving their quality of life. The collection of information was carried out using data present in the patients' medical records, as well as the use of a recording method called SOAP (Subjective, Objective, Assessment, Plane), which allowed the collection and organization of patient data, the identification of problems related to pharmacotherapy, the elaboration of a care plan together with the patient. The present work found that in the pharmacotherapies used by the elderly there were three mild interactions, twenty one moderate interactions and fourteen severe interactions and the presence of undesirable signs and symptoms in the elderly, which may be associated with the use of some medications or untreated pathologies. It also found the use of several medications that are inappropriate for the elderly according to the 2015 Beers Criteria. In view of these findings, he sent a letter to the prescriber describing the problems encountered in order to achieve a more effective pharmacotherapy.

A população de idosos é a que mais possui doenças, por consequência é a que mais utiliza os serviços de saúde. Estima-se que quanto mais medicamentos o idoso fizer uso, maior o risco de ter uma interação medicamentosa. O objetivo do presente estudo foi realizar o acompanhamento farmacoterapêutico de idosos visando a melhoria na sua qualidade de vida. A coleta de informações foi realizada utilizando de dados presentes no prontuário dos pacientes, bem como da utilização de um método de registro denominado SOAP (Subjective, Objective, Assessment, Plane), este permitiu a coleta e organização de dados do paciente, a identificação dos problemas relacionados à farmacoterapia, a elaboração de um plano de cuidado em conjunto com o paciente. O presente trabalho constatou que nas farmacoterapias utilizadas pelo idosos havia três interações leves, vinte uma interações moderadas e quatorze interações graves e a presença de sinais e sintomas indesejáveis nos idosos, que pode estar associada ao uso de alguns medicamentos ou a patologias não tratadas. Também constatou o uso de diversos medicamentos que são inapropriados aos idosos de acordo com os Critérios de Beers 2015.
