ACOMPANHAMENTO FARMACOTERAPÊUTICO DE PACIENTES IDOSOS DIABÉTICOS.

Resumo: Pharmacotherapeutic monitoring makes it possible to work with individual patients, groups or families to investigate everything from access to medication to adherence to treatment, whether in terms of administration, adverse reactions or drug interactions. This research on Pharmacotherapeutic Monitoring is aimed at elderly patients diagnosed with Diabetes Mellitus. The follow-up was aimed at a group of diabetic patients residing in a Long Stay Institution for the Elderly, in the city of Mococa, São Paulo. Drug interactions, adverse drug reactions and surveys on the use of polypharmacy among the elderly were analyzed. The aim of this study was to carry out pharmacotherapeutic monitoring of patients diagnosed with Diabetes Mellitus, identifying problems related to medications and possible adverse reactions, in order to provide patients with a better quality of life. Information was collected using data from the patients' medical records, as well as a recording method called SOAP (Subjective, Objective, Assessment, Plan.) Through the SOAP method in the pharmacotherapies used by the elderly, thirteen moderate interactions and sixteen interactions were identified serious. Also in this study, the use of several medications that are inappropriate for the elderly according to the criteria of Beers (2015) was found.

Esta pesquisa em Acompanhamento Farmacoterapêutico é voltada a pacientes idosos diagnosticados com Diabetes Melitos. O acompanhamento foi dirigido a um grupo de pacientes diabéticos que reside em uma Instituição de Longa Permanência para Idosos, na cidade de Mococa, São Paulo. Foram analisadas interações medicamentosas, reações adversas por medicamentos e levantamentos do uso de polifarmácia entre os idosos. O objetivo do presente trabalho foi realizar o acompanhamento farmacoterapêutico de pacientes diagnosticados com Diabetes Melitos, identificando os problemas relacionados a medicamentos e as possíveis reações adversas, para, assim, proporcionar uma qualidade de vida melhor aos pacientes. A coleta de informações foi realizada utilizando dados presentes no prontuário dos pacientes, bem como um método de registro denominado SOAP (Subjective, Objective, Assessment, Plan.) Através do método SOAP nas farmacoterapias utilizadas pelo idosos, foram identificadas treze interações moderadas e dezesseis interações graves.
