AVALIAÇÃO DA SOBREVIDA DE IMPLANTES EM MAXILAS ATRÓFICAS COM E SEM ENXERTO ÓSSEO: UMA REVISÃO SISTEMÁTICA.

Resumo: The reduction of alveolar ridge is a progressive condition of multifactorial and irreversible origin that affects many people. it is essential for the success of rehabilitation with implants a correct planning with an appropriate surgical technique for each case.  The present systematic review evaluated whether the performance of a surgical technique with bone grafting influences the survival of implants installed in atrophic maxillae. The systematic review of the literature was developed following the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses PRISMA, with modifications, and the research question was elaborated according to the "peaks" strategy: population, intervention, comparison, outcome and types of studies. Between 2017 and 2022, a total of 315 abstracts in Portuguese and English were identified in the Medline databases via Pubmed, BBO and Scielo. After selection and evaluation, 11 full articles (primary studies) - randomized clinical trials and clinical case reports - were selected, compared and data extraction performed by two independent reviewers.  Among the surgical techniques applied to gain bone height in implant rehabilitation in atrophic maxillae (moderate to severe) 39% of the studies chose to apply a surgical technique with bone grafting and 61% without bone grafting. Both techniques were associated with a 90-100% implant survival rate with little or no post-surgical complications. It is concluded that the occurrence of failures is small and that the use of bone grafts does not tend to increase implant survival.

It is concluded that the occurrence of failures is small and that the use of bone grafts does not tend to increase implant survival.    A diminuição dos rebordos alveolares é uma condição progressiva de origem multifatorial e irreversível que afeta muitas pessoas. é imprescindível para o sucesso reabilitador com implantes um correto planejamento com uma técnica cirúrgica adequada para cada caso.  A presente revisão sistemática avaliou se a realização de técnica cirúrgica com enxerto ósseo influencia a sobrevida de implantes instalados em maxilas atróficas. A revisão sistemática da literatura foi desenvolvida seguindo as diretrizes dos do preferrend reporting items for systematic reviews and metaanalyses PRISMA, com modificações e a pergunta de pesquisa foi elaborada de acordo a estratégia “picos”: população, intervenção, comparação, resultado e tipos de estudos. Entre os anos de 2017 e 2022, identificou-se nas bases de dados Medline via Pubmed, BBO e Scielo um total de 315 resumos nas línguas portuguesa e inglesa. Após processo seletivo e avaliação 11 artigos completos (estudos primários) – ensaios clínicos randomizados e relato de caso clínico – foram selecionados, comparados e a extração de dados realizada por dois revisores independentes.  Dentre as técnicas cirúrgicas aplicadas para ganhar altura óssea na reabilitação com implantes em maxilas atróficas (moderada a severa) 39% dos estudos optaram por aplicar uma técnica cirúrgica com enxerto ósseo e 61% sem enxerto ósseo. Ambas as técnicas se associaram a uma taxa de sobrevida dos implantes de 90 a 100% tendo com pouca ou nenhuma complicação pós-cirúrgica.
