OVERWEIGHT AND HYPERTENSIVE SYNDROMES DURING PREGNANCY: REALITY IN A BRAZILIAN HOSPITAL

Resumo: Objective: The aim of the present study was to test associations between pregnancy-induced hypertensive syndromes (PIHSs) and nutritional, obstetric, clinical and socioeconomic characteristics in high-risk pregnancies. Methods: A cross-sectional study was carried out involving 191 pregnant women (mean age: 24.7 ± 6.17 years) at the obstetric center of the Federal University of Pernambuco Hospital (Brazil) between March and November 2013. Information was collected on the diagnosis of PIHS, pre-pregnancy anthropometric data (weight and height), number of prenatal appointments, clinical data (previous arterial hypertension, family history of hypertension and associated adverse health conditions), obstetric data (number of pregnancies, type of childbirth and past history of PIHS), lifestyle (smoking, alcohol intake and physical activity) and income. Statistical analysis involved the chi-square test with the level of significance set to 5% (p < 0.05). Results: An increase in the frequency of preeclampsia was found among all PIHSs. Overweight/obesity was significantly associated with gestational hypertension, preeclampsia (p < 0.054) and a non-fixed income (p < 0.035). The most prevalent complication related to PIHS was the premature rupture of membranes with the loss of amniotic fluid. A greater percentage of a family history of hypertension, primiparity and sedentary lifestyle occurred with all PIHSs. No significant associations were found between the different types of PIHS and the other variables studied. Conclusion: Preeclampsia and gestational hypertension were significantly associated with overweight/obesity and a non-fixed income.

Métodos: Estudo transversal realizado no período março/novembro de 2013, com 191 gestantes de idade média de 24,7 +6,17 anos, do centro obstétrico do Hospital das Clínicas da UFPE, onde foram coletados os diagnósticos da SHEG, dados antropométricos pré-gestacionais (peso e altura), número de consultas pré-natal, dados clínicos (hipertensão arterial prévia, histórico familiar de hipertensão e patologias associadas), obstétricos (número de gestações e abortos, tipos de parto e presença de SHEG anterior) estilo de vida (tabagismo, etilismo  e  atividade física) e renda. No tratamento estatistico foi aplicado o teste de Qui-quadrado com nível de significancia estatistica p<0,05. Resultados: Na amostra estudada houve aumento da frequência da Pré-eclâmpsia (PE) entre todas as SHEG. Observou-se relação positiva do sobrepeso/obesidade com Hipertensão Gestacional (HG) e PE (p<0,054) e renda não fixa (p<0,035). Ocorreram complicações patologicas  relacionadas a SHEG, sendo mais prevalente a ruptura prematura de menbranas com perda de líquido amniótico. O histórico familiar de hipertensão, primiparidade e sedentarismo ocorreram com maior percentual em todas as síndromes. Os demais fatores estudados não apresentaram associação com os diferentes tipos de SHEG.
