Treatment of acute post-surgical bacterial infection in sinus elevation: Case report.

Resumo: Introduction: Infection of the sinus graft with consequent acute bacterial rhinosinusitis (ABRS) is a complication that can occur during the postoperative period of sinus augmentation surgery. A small group of bacteria seems to predominate in ABRS, and these bacteria are empirically susceptible to tetracycline antibiotics, preventing the presence of resistant strains. Historically, clinicians have mixed tetracycline with various biomaterials or hydrated biomaterials in a diluted solution of tetracycline in the treatment of periodontal disease and for crest preservation / enlargement indications. Case presentation: In the present case, a 39-year-old African American man with sinus graft infection and signs and symptoms consistent with ABRS was successfully treated by removing the initial material from the graft, completely debriding and irrigating the antrum and placing a lyophilized allograft bone hydrated in an aqueous solution of tetracycline. Conclusion: Given the typical bacteria present in ABRS, tetracycline applied locally can be useful prophylactically in sinus lift surgery or as an adjunct in the treatment of postoperative sinus graft infection.

Apresentação do caso: No presente caso, um homem afro-americano de 39 anos de idade com infecção do enxerto sinusal e sinais e sintomas consistentes com o ABRS foi tratado com sucesso removendo o material inicial do enxerto, desbridando e irrigando completamente o antro e colocando um osso liofilizado aloenxerto hidratado em uma solução aquosa de tetraciclina.
