THE IMPORTANCE OF PHYSIOTHERAPY IN REVERSING ATELECTASIA IN PRETERM NEWBORN IN THE NEONATAL INTENSIVE CARE UNIT.

Resumo: | Introduction: Atelectasis is a pulmonary dysfunction, where partial or total alveolar collapse occurs in one or more segments of the lung. PTNBs (Pre-Term Newborns) are classified by birth below 37 weeks of gestational age. Preterm infants tend to develop atelectasis in the first days of life and throughout their training, due to the incomplete anatomophysiological formation of the Respiratory System. Preterm premature babies have a smaller number of alveolar units, underdevelopment or absence of contralateral ventilation, insufficient surfactant production, resulting in a compromised ventilation/perfusion relationship. Respiratory physiotherapy is essential within the Neonatal Intensive Care Unit, acting in the prevention, reversal and care of patients with complications and pulmonary dysfunctions. Method: This study aims to systematically review the available literature, and demonstrate the importance of physiotherapy in reversing atelectasis within the Neonatal Intensive Care Unit in PTNB. Results: The review was carried out through eligible studies found in the Pubmed and Scielo databases between the years 2017 to 2023. Discussion: The results brought into discussion by the authors, exclusively physiotherapeutic techniques, such as increased expiratory flow, contralateral pulmonary ventilation, positioning global, use of continuous positive airway pressure and alveolar recruitment, with the purpose of expanding collapsed alveolar areas, reestablishing the ventilation/perfusion relationship, restoring lung volumes. Conclusion: Therefore, the role of physiotherapy is fundamental within this scenario, through manual and/or mechanical techniques, which result in lung expansion, providing comfort to the PTNB and favoring hematosis. |

Los RNPT (recién nacidos prematuros) se clasifican por nacimiento con menos de 37 semanas de edad gestacional. Los bebés prematuros tienden a desarrollar atelectasias en los primeros días de vida y durante toda su formación, debido a la formación anatomofisiológica incompleta del Sistema Respiratorio. Los recién nacidos prematuros tienen un menor número de unidades alveolares, subdesarrollo o ausencia de ventilación contralateral, producción insuficiente de surfactante, lo que resulta en una relación ventilación/perfusión comprometida. La fisioterapia respiratoria es necesaria dentro de la Unidad de Cuidados Intensivos Neonatales, actuando en la prevención, reversión y atención de pacientes con complicaciones y disfunciones pulmonares. Método: Este estudio tiene como objetivo revisar sistemáticamente la literatura disponible y demostrar la importancia de la fisioterapia para revertir la atelectasia dentro de la Unidad de Cuidados Intensivos Neonatales en RNPT. Resultados: La revisión se realizó a través de estudios elegibles encontrados en las bases de datos Pubmed y Scielo entre los años 2017 a 2023. Discusión: Los resultados puestos en discusión por los autores, técnicas exclusivamente fisioterapéuticas, como aumento del flujo espiratorio, ventilación pulmonar contralateral, posicionamiento global, uso de presión positiva continua en las vías respiratorias y reclutamiento alveolar, con el propósito de expandir las áreas alveolares colapsadas, restablecer la relación ventilación/perfusión, restaurar los volúmenes pulmonares. Conclusión: Por lo tanto, el papel de la fisioterapia es fundamental en este escenario, a través de técnicas manuales y/o mecánicas, resultando en expansión pulmonar, comodidad para el RNPT y favoreciendo la hematosis.Introdução: A atelectasia é uma disfunção pulmonar, onde ocorre o colapso alveolar parcial ou total, de um ou mais segmentos do pulmão. Os RNPT (Recém Nascidos Pré Termo) são classificados por nascimento abaixo de 37 semanas de idade gestacional. Os RNPT tendem a desenvolver atelectasias nos primeiros dias de vida e no decorrer de sua formação, devido a formação incompleta anatomofisiológica do Sistema Respiratório. Os prematuros pré termo possuem menor número de unidades alveolares, subdesenvolvimento ou ausência de ventilação contralateral, produção de surfactante insuficiente, resultando na relação ventilação/perfusão comprometida. A fisioterapia respiratória é imprescindível dentro da Unidade de Terapia Intensiva Neonatal, atuando na prevenção, reversão e cuidados sob o paciente com complicações e disfunções pulmonares. Método: Este estudo tem como objetivo revisar sistematicamente dentre a literatura disponível, e demonstrar a importância da fisioterapia na reversão de atelectasias dentro da Unidade de Terapia Intensiva Neonatal em RNPT.
