EPIDEMIOLOGICAL PROFILE OF CONGENITAL SYPHILIS IN THE STATE OF TOCANTINS IN THE YEAR 2018

Summary: Introduction: Congenital syphilis is a bacterial disease whose etiological agent is Treponema pallidum. It is transmitted to the fetus when the pregnant woman is infected or has undergone inappropriate treatment. Transmission can be vertical (transplacental) or during labor. It has a very variable clinical picture, and can be classified as early or late congenital syphilis. Objective: To describe the epidemiological profile of cases of congenital syphilis in the state of Tocantins in 2018. Methodology: This is an epidemiological, descriptive, quantitative and retrospective study, based on secondary data, extracted from SINAN-Net. Results: In 2018, 286 cases of congenital syphilis were confirmed in the state of Tocantins, 151 of which were in male newborns and 135 in females. Regarding race, 90.55% of cases were in brown patients. Regarding maternal education, 76 reported cases were children of mothers with incomplete primary education and 60 of them had incomplete secondary education. Most cases of congenital syphilis were transmitted transplacentally, with 182 cases (63.63%) reported during prenatal care and 92 during labor. Among the 286 cases reported, 13 mothers did not undergo prenatal care and 66.08% of partners did not undergo treatment for syphilis. Conclusion: In view of this, it is concluded that, among the factors that increase the incidence of congenital syphilis in Tocantins, the low level of education of mothers stands out, and, in most cases, there was no treatment for their partners. Therefore, measures such as disseminating guidance on syphilis in primary health care, associated with training professionals in the area, are crucial to reducing cases of the disease.

Se transmite al feto cuando la mujer embarazada está infectada o ha recibido un tratamiento inadecuado. La transmisión puede ser vertical (transplacentaria) o durante el parto. Tiene un cuadro clínico muy variable, pudiendo clasificarse como sífilis congénita temprana o tardía. Objetivo: Describir el perfil epidemiológico de los casos de sífilis congénita en el estado de Tocantins en 2018. Metodología: Se trata de un estudio epidemiológico, descriptivo, cuantitativo y retrospectivo, basado en datos secundarios, extraídos del SINAN-Net. Resultados: En 2018, se confirmaron 286 casos de sífilis congénita en el estado de Tocantins, de los cuales 151 en recién nacidos varones y 135 en niñas. En cuanto a la raza, el 90,55% de los casos fueron en pacientes pardos. En cuanto a la educación materna, 76 casos reportados fueron hijos de madres con educación primaria incompleta y 60 de ellas tenían educación secundaria incompleta. La mayoría de los casos de sífilis congénita se transmitieron por vía transplacentaria, notificándose 182 casos (63,63%) durante la atención prenatal y 92 durante el parto. Entre los 286 casos reportados, 13 madres no realizaron control prenatal y el 66,08% de las parejas no recibieron tratamiento para la sífilis. Conclusión: Ante esto, se concluye que, entre los factores que aumentan la incidencia de sífilis congénita en Tocantins, se destaca el bajo nivel de escolaridad de las madres y, en la mayoría de los casos, no hubo tratamiento para sus parejas. Por lo tanto, medidas como la difusión de orientaciones sobre sífilis en la atención primaria de salud, asociadas a la capacitación de profesionales en el área, son cruciales para reducir los casos de la enfermedad.Introdução:A sífilis congênita é uma doença bacteriana que tem como agente etiológico o Treponema pallidum. Possui quadro clínico bastante variável, e pode ser classificada como sífilis congênita precoce ou tardia. Objetivo: Descrever o perfil epidemiológico de casos de sífilis congênita no estado do Tocantins no ano de 2018. Metodologia: Trata-se de um estudo epidemiológico, descritivo, de caráter quantitativo e retrospectivo, pautado em dados secundários, extraídos do SINAN-Net. Resultados: No ano de 2018, foram confirmados 286 casos de sífilis congênita no estado do Tocantins, sendo 151 deles em neonatos do sexo masculino e 135 no sexo feminino. Quanto à raça, 90,55% dos casos foram em pacientes pardos. Em relação à escolaridade materna, 76 casos notificados eram filhos de mães de ensino fundamental incompleto e 60 delas possuíam ensino médio incompleto. A maioria dos casos de sífilis congênita foram transmitidos via transplacentária, sendo 182 casos (63,63%)  notificados durante o pré-natal e 92 no trabalho de parto. Dentre os 286 casos notificados, 13 mães não realizaram o pré-natal e 66,08% dos parceiros não realizaram tratamento para sífilis. Conclusão: Diante disso, conclui-se que, dentre os fatores que aumentam a incidência de sífilis congênita no Tocantins, destaca-se a baixa escolaridade das mães, e, na maioria dos casos, não houve tratamento dos parceiros.
