Psoriasis, comorbidities, therapy and its adverse effects

Resumo: Psoriasis is a chronic inflammatory skin disease that affects 2-3% of the population worldwide, is characterized by erythematous scaly plaques and can involve the joints in the case of arthropathic psoriasis and is associated with significant impairments in physical and mental health due to its impact on daily activities. Objectives: To discuss the comorbidities and treatment of psoriasis and its adverse effects. Methodology: This is a literature review with a qualitative descriptive approach. The search for material took place in September 2023, using the SciELO and PubMed databases. For the search, the following terms (keywords and delimiters) will be used in combination with the "AND" connective, using the following descriptors: "Psoriasis"; "Comorbidity"; "Treatment". Results: Studies have shown that patients with psoriasis have increased hyperlipidemia, hypertension, coronary artery disease, type 2 diabetes and increased body mass index. Therapeutic options for psoriasis include topical therapy, phototherapy or systemic treatment. Mild to moderate psoriasis, for example, can be treated topically with a combination of therapies, while moderate to severe psoriasis usually requires systemic treatment; and the presence of comorbidities such as psoriatic arthritis is also highly relevant in treatment management. Advances in diagnosis and treatment have resulted in a growing number of therapeutic options that can dramatically improve the lives of people with psoriasis. However, the disease is still very common, and treatment and access do not yet meet patients' needs.

Objetivos: Discutir las comorbilidades y el tratamiento de la psoriasis y sus efectos adversos. Metodología: Se trata de una revisión bibliográfica con un enfoque cualitativo descriptivo. La búsqueda de material se realizó en septiembre de 2023, utilizando las bases de datos SciELO y PubMed. Para la búsqueda se utilizarán los siguientes términos (palabras clave y delimitadores), combinándolos mediante la conectiva "AND", utilizando los siguientes descriptores: "Psoriasis"; "Comorbidity"; "Treatment". Resultados: Los estudios han demostrado que los pacientes con psoriasis presentan un aumento de la hiperlipidemia, la hipertensión, la enfermedad coronaria, la diabetes de tipo 2 y un mayor índice de masa corporal. Las opciones terapéuticas para la psoriasis incluyen la terapia tópica, la fototerapia o el tratamiento sistémico. La psoriasis de leve a moderada, por ejemplo, puede tratarse tópicamente con una combinación de terapias, mientras que la psoriasis de moderada a grave suele requerir tratamiento sistémico; y la presencia de comorbilidades como la artritis psoriásica también es muy relevante en la gestión del tratamiento. Los avances en el diagnóstico y el tratamiento han dado lugar a un número creciente de opciones terapéuticas que pueden mejorar notablemente la vida de los enfermos de psoriasis. Sin embargo, la enfermedad sigue siendo muy común, y el tratamiento y el acceso a él no satisfacen aún las necesidades de los pacientes.A psoríase é uma doença inflamatória crônica da pele que afeta 2–3% da população em todo o mundo, é caracterizada por placas eritematosas escamosas e pode envolver as articulações no caso de psoríase artropática, e está associada a prejuízos significativos na saúde física e mental devido ao seu impacto nas atividades diárias. Objetivos: discutir as comorbidades e tratamento para psoríase e seus efeitos adversos. Metodologia: Trata-se de uma revisão bibliográfica, com abordagem descritiva qualitativa. A busca de material ocorreu no mês de setembro de 2023, a partir em base de dados da SciELO e PubMed. Para a busca serão utilizados os seguintes termos (palavras-chaves e delimitadores) combinações dos mesmos através do conectivo “AND”, utilizando os seguintes descritores: “Psoriasis”; “Comorbidity”; “Treatment”. Resultados: Estudos revelaram que os pacientes com psoríase apresentam aumento de hiperlipidemia, hipertensão, doença arterial coronariana, diabetes tipo 2 e aumento do índice de massa corporal. As opções terapêuticas para a psoríase incluem terapia tópica, fototerapia ou tratamento sistêmico. A psoríase leve a moderada, por exemplo, pode ser tratada topicamente com uma combinação de terapia, enquanto a moderada a grave geralmente requer tratamento sistêmico; e a presença de comorbidades como artrite psoriásica também é altamente relevante no manejo do tratamento. Os avanços nos diagnósticos e tratamento resultaram em um número crescente de opções terapêuticas que podem melhorar dramaticamente a vida das pessoas com psoríase.
