Approach to abdominal pain in the emergency department: importance of imaging studies in the differential diagnosis of acute abdomen

Resumo: | Abdominal pain is one of the most common complaints in patients seeking care in emergency departments, representing a diagnostic challenge due to its diverse etiology. In this context, this article aims to highlight the importance of imaging studies in the differential diagnosis of acute abdomen, a condition that requires rapid identification and treatment. Through a literature review, this study addresses the potential causes of acute abdominal pain, from benign conditions to life-threatening emergencies, and how imaging studies contribute decisively in differentiating these conditions. The aim is to evaluate the efficacy of different imaging modalities, such as radiography, ultrasonography, computed tomography, and magnetic resonance imaging, in identifying specific causes of acute abdomen. The methodology employed involved the analysis of relevant studies and articles published in reliable databases, addressing both the diagnostic accuracy and the speed with which each type of imaging test can contribute to the diagnosis. The results show that, while some conditions can be identified through simpler and less expensive imaging methods, others require more advanced and detailed techniques for an accurate diagnosis. It is concluded that imaging studies act in the management of patients with abdominal pain in the emergency department, speeding up diagnosis and treatment, reducing the likelihood of serious complications through an accurate and early identification of the underlying causes of acute abdomen. |

El dolor abdominal es una de las quejas más frecuentes en los pacientes que acuden a urgencias, representando un reto diagnóstico debido a su diversa etiología. En este contexto, este artículo tiene como objetivo resaltar la importancia de los estudios de imagen en el diagnóstico diferencial del abdomen agudo, una condición que requiere una rápida identificación y tratamiento. A través de una revisión bibliográfica, este estudio aborda las posibles causas del dolor abdominal agudo, desde condiciones benignas hasta emergencias potencialmente mortales, y cómo los estudios de imagen contribuyen decisivamente a diferenciar estas condiciones. El objetivo es evaluar la eficacia de diferentes modalidades de imagen, como la radiografía, la ecografía, la tomografía computarizada y la resonancia magnética, en la identificación de causas específicas de abdomen agudo. La metodología empleada consistió en el análisis de estudios relevantes y artículos publicados en bases de datos confiables, abordando tanto la precisión diagnóstica como la rapidez con la que cada tipo de prueba de imagen puede contribuir al diagnóstico. Los resultados muestran que, si bien algunas afecciones pueden identificarse a través de métodos de imagen más simples y menos costosos, otras requieren técnicas más avanzadas y detalladas para un diagnóstico preciso. Se concluye que los estudios imagenológicos actúan en el manejo de los pacientes con dolor abdominal en el servicio de urgencias, acelerando el diagnóstico y tratamiento, reduciendo la probabilidad de complicaciones graves a través de una identificación precisa y precoz de las causas subyacentes del abdomen agudo. A dor abdominal é uma das queixas mais comuns em pacientes que buscam atendimento em departamentos de emergência, representando um desafio diagnóstico devido à sua etiologia diversa. Neste contexto, este artigo visa destacar a importância dos exames de imagem no diagnóstico diferencial de abdome agudo, uma condição que necessita de rápida identificação e tratamento. Através de uma revisão de literatura, este estudo aborda as causas potenciais de dor abdominal aguda, desde condições benignas até emergências que ameaçam a vida, e como os exames de imagem contribuem decisivamente na diferenciação dessas condições. Busca-se avaliar a eficácia de diferentes modalidades de imagem, como radiografia, ultrassonografia, tomografia computadorizada e ressonância magnética, na identificação de causas específicas de abdome agudo. A metodologia empregada envolveu a análise de estudos e artigos relevantes publicados em bases de dados confiáveis, abordando tanto a precisão diagnóstica quanto a rapidez com que cada tipo de exame de imagem pode contribuir para o diagnóstico. Os resultados obtidos demonstram que, enquanto algumas condições podem ser identificadas por meio de métodos de imagem mais simples e menos custosos, outras exigem técnicas mais avançadas e detalhadas para um diagnóstico preciso.
