PRENATAL CARE IN THE STATE OF PERNAMBUCO: A COMPARATIVE STUDY OF INQUIRIES

Resumo: | Prenatal care stands out as an essential factor in the protection and prevention of adverse events on the health of the mother and child, allowing the identification and clinical management of timely interventions on potential risk factors for health complications. Our article aims at describing and comparing the main characteristics of prenatal care in 1997 and 2006 in the state of Pernambuco, Brazil. A population-based, observational, cross-sectional, study was conducted using secondary data from 1997 and 2007 extracted from the State Research on Health and Research Databases (II and III). In 2006, prenatal was considered satisfactory if it met the three minimum recommendations of the Ministry of Health: starting prenatal care until the 16th week of pregnancy, provisioning of six or more consultations, and offering guidance on the reasons and practice of breastfeeding. Prenatal care increased significantly in 2006 (17.1%) compared to 1997. Prenatal care in 2006 was more satisfying for women that were 20 to 35 years old (60.7%), white (60.0%); with greater schooling (71.8%), income equal to or greater than minimum wage (75.8%), with two to three children (63.1%), and that resided in the Recife Metropolitan Region (68.3%). Prenatal care was less satisfying for women that worked (54.7%) and from the rural area (49.1%, p < 0.001). Despite the expansion of prenatal coverage, improving its quality is essential to promote holistic health, especially in poor communities, where risk factors are prevalent. |

Se propone describir y comparar las principales características de la asistencia prenatal en los años 1997 y 2006 en el estado de Pernambuco. Se llevó a cabo un estudio de base poblacional, observacional, de cohorte transversal, con el uso de datos secundarios que se extrajeron de las bases de datos de la segunda y la tercera Encuestas Estaduales de la Salud y la Nutrición, que se habían llevado a cabo entre 1997 y 2006, respectivamente. Se consideró satisfactoria, en 2006, la asistencia prenatal que había cumplido tres de los requisitos mínimos preconizados por el Ministerio de la Salud: inicio del prenatal hasta la 16.ª semana del embarazo, seis o más citas y orientaciones sobre las razones y la práctica de la lactancia materna. Hubo una importante mejora en la asistencia prenatal en 2006 (aumento del 17,1%) en comparación con el 1997. El prenatal en el 2006 fue más satisfactorio para las mujeres de entre 20 y 35 años (60,7%), blancas (60,0%), con mayor nivel de estudios (71,8%), con ingresos igual o superior a 1 sueldo mínimo (75,8), con dos o tres hijos (63,1%) y moradores en la región metropolitana de Recife (68,3%). Sin embargo, fue menos satisfactorio para las mujeres que trabajan (54,7%) y que viven en el campo (49,1%, p < 0,001). A pesar de la ampliación de la cobertura prenatal, es necesario mejorar su calidad con el fin de promover la salud integrada, sobre todo en comunidades menos desfavorecidas donde prevalecen más los factores de riesgo.A assistência pré-natal se destaca como fator essencial na proteção e prevenção a eventos adversos sobre a saúde do binômio mãe e filho, possibilitando a identificação e o manuseio clínico de intervenções oportunas sobre potenciais fatores de risco para complicações à saúde. Objetiva-se descrever e comparar as principais características da assistência pré-natal nos anos de 1997 e 2006, no estado do Pernambuco. Realizou-se estudo de base populacional, observacional, de coorte transversal, utilizando dados secundários, extraídos de informações dos bancos de dados das segunda e terceira Pesquisas Estaduais de Saúde e Nutrição, realizadas nos anos de 1997 e 2006, respectivamente. Considerou- -se satisfatória, em 2006, a assistência pré-natal que atendeu três das recomendações mínimas preconizadas pelo Ministério da Saúde: início do pré-natal até a 16ª semana de gestação, seis ou mais consultas e orientações sobre razões e prática do aleitamento materno. Houve importante melhora na assistência pré-natal em 2006 (acréscimo de 17,1%) quando comparada a 1997. O pré-natal no ano de 2006 foi mais satisfatório para as mulheres com 20 a 35 anos (60,7%), brancas (60,0%), de maior escolaridade (71,8%), com renda superior ou igual a um salário-mínimo (75,8%), com dois a três filhos (63,1%) e residentes na Região Metropolitana do Recife (68,3%). Apresentou-se menos satisfatório para as mulheres trabalhadoras (54,7%) e do interior rural (49,1%, p < 0,001).
