Prenatal care by the family health strategy in a reference city of northeastern Brazil

Resumo: Prenatal care plays an important role in maternal and child care and, when qualified, can prevent complications related to pregnancy. The quality of prenatal care in the Family Health Strategy was evaluated in a macro-regional reference municipality in the Brazilian Northeast. This is a normative assessment with an analytical level centered on the Family Health Teams (EqSF). Structured interviews were carried out with nurse managers of the EqSF, through a structured questionnaire, validated nationally and with evaluative standards of the structure and work process of the EqSF, centered on two dimensions of analysis: care and management. The data were processed and analyzed in a descriptive way, considering the frequency of adaptation of the teams to each evaluative standard. 69% (9) of the evaluated EqSF met the quality standards in prenatal care satisfactorily and 31% (4) met it regularly. The percentage of adequacy of the teams evaluated in the studied scenario was higher than that found in other municipalities in Brazil, where the adequacy has shown an average ranging between 15% and 40%. In the managerial dimension, the attributes of work processes were the most critical of adequacy and in the care dimension, the attributes of structure had greater difficulty in achieving adequacy to the quality standards of prenatal care. This study helped to identify the main obstacles and potentialities of the structure and management and care processes of prenatal care. Their findings revealed which are the most critical standards of adequacy and which demand from municipal managers and local managers quality improvement strategies in prenatal care.

Se evaluó la calidad de la atención prenatal en la Estrategia Salud de la Familia en un municipio de referencia macrorregional del Nordeste brasileño. Esta es una evaluación normativa con un nivel analítico centrado en los Equipos de Salud de la Familia (EqSF). Se realizaron entrevistas estructuradas con enfermeros gestores del EqSF, a través de un cuestionario estructurado, validado a nivel nacional y con estándares evaluativos de la estructura y proceso de trabajo del EqSF, centrado en dos dimensiones de análisis: cuidado y gestión. Los datos fueron procesados ??y analizados de forma descriptiva, considerando la frecuencia de adaptación de los equipos a cada estándar evaluativo. El 69% (9) de los EqSF evaluados cumplieron satisfactoriamente con los estándares de calidad en la atención prenatal y el 31% (4) cumplieron regularmente. El porcentaje de adecuación de los equipos evaluados en el escenario estudiado fue superior al encontrado en otros municipios de Brasil, donde la adecuación ha mostrado una media que oscila entre el 15% y el 40%. En la dimensión gerencial, los atributos de los procesos de trabajo fueron los más críticos de adecuación y en la dimensión asistencial, los atributos de estructura tuvieron mayor dificultad para lograr la adecuación a los estándares de calidad de la atención prenatal. Este estudio ayudó a identificar los principales obstáculos y potencialidades de la estructura y los procesos de gestión y atención del prenatal. Sus hallazgos revelaron cuáles son los estándares de adecuación más críticos y cuáles demandan de los gestores municipales y locales estrategias de mejora de la calidad en la atención prenatal.O pré-natal tem um importante papel na atenção materno-infantil e quando qualificado pode prevenir complicações ligadas à gestação. Na dimensão gerencial, os atributos de processos de trabalho foram os mais críticos de adequação e na dimensão assistencial os atributos de estrutura tiveram maior dificuldade de alcançar a adequação aos padrões de qualidade do pré-natal. Este estudo contribuiu para identificar os principais obstáculos e potencialidades da estrutura e processos gerenciais e assistenciais da atenção pré-natal.
