Social determinants and mortality from tuberculosis in Brazil: a literature review

Resumo: | Tuberculosis is an infectious bacterial disease that affects the lungs, being the leading cause of death by a single infectious disease agent worldwide. Some studies have found an association between the social determinants of health and the occurrence of tuberculosis. However, there are no reports of reviews that have assessed the relationship between the social determinants of health and mortality from tuberculosis in the Brazilian population. This study aimed to describe the main social determinants related to tuberculosis mortality in the Brazilian population. We conducted a narrative review in the MEDLINE, Lilacs, Scielo, and Web of Science databases. Advanced age, low income, low education, and HIV and tuberculosis co-infection were the main determinants of tuberculosis mortality in Brazil. Undesirable drug interactions, the difficulty for the elderly to report the symptoms of the disease, abandonment of treatment, and inequalities in access to health systems are some of the causes that are related to social determinants and mortality from tuberculosis. Our findings showed that advanced age, HIV and tuberculosis co-infection, low income, and low education were the social determinants most related to tuberculosis mortality. Public policies should be directed at individuals who are more vulnerable to tuberculosis and more likely to die from the disease. |

Algunos estudios han encontrado una asociación entre los determinantes sociales de la salud y la aparición de tuberculosis. Sin embargo, no existen informes de revisiones que hayan evaluado la relación entre los determinantes sociales de la salud y la mortalidad por tuberculosis en la población brasileña. Este estudio tuvo como objetivo describir los principales determinantes sociales relacionados con la mortalidad por tuberculosis en la población brasileña. Se trató de una revisión narrativa, realizada en las bases de datos MEDLINE, Lilacs, SciELO y Web of Science. Las interacciones medicamentosas indeseables, la dificultad de los ancianos para informar los síntomas de la enfermedad, el abandono del tratamiento y las desigualdades para acceder a los sistemas de salud son algunas de las causas que se relacionan con los determinantes sociales y la mortalidad por tuberculosis. Nuestros hallazgos mostraron que la edad avanzada, la coinfección por el virus de la inmunodeficiencia humana (VIH) y la tuberculosis, los bajos ingresos y la baja educación fueron los determinantes sociales más relacionados con la mortalidad por tuberculosis en Brasil. Las políticas públicas deben estar dirigidas a las personas que son más vulnerables a la tuberculosis y a los riesgos de fallecimiento a causa de la enfermedad.A tuberculose é uma doença bacteriana infecciosa que afeta os pulmões, sendo a principal causa de morte por um único agente de doença infecciosa no mundo. Alguns estudos encontraram associação entre os determinantes sociais da saúde e a ocorrência de tuberculose. No entanto, não há relatos de revisões que tenham avaliado a relação entre os determinantes sociais da saúde e a mortalidade por tuberculose na população brasileira. Este estudo teve como objetivo descrever os principais determinantes sociais que estão relacionados com a mortalidade por tuberculose na população brasileira. Trata-se de uma revisão narrativa, realizada nas bases de dados do Medline, Lilacs, Scielo e Web of Science. Interações medicamentosas indesejáveis, dificuldade de o idoso relatar os sintomas da doença, abandono do tratamento e desigualdades no acesso aos sistemas de saúde são algumas causas relacionadas aos determinantes sociais e à mortalidade por tuberculose. Nossos achados mostraram que idade avançada, coinfecção pelo vírus da imunodeficiência humana (HIV) e por tuberculose, baixa renda e baixa escolaridade foram os determinantes sociais mais relacionados à mortalidade por tuberculose no Brasil.
