Evaluation of ASC/SIL Ratio Utility as a Cytopathology Quality Marker

Resumo: | Introduction: The term “atypical squamous cells” (ASC) is considered the most frequently epithelial abnormality described in cervical smears, and it’s related to an uncertainty epidemiological group. The ASC/SIL (squamous intraepithelial lesion) ratio has been used by many cytopathology laboratories as a quality marker. Objective: To evaluate the annual ASC/SIL ratio in a large Surgical Pathology Laboratory for a period of 10 years, comparing these data to the values recommended by the College of American Pathologists (CAP), and to verify the utility of the ASC/SIL ratio as a quality marker tool in the ASC cytological interpretation for any cytopathology laboratory. Method: Retrospective and transversal study (2002 to 2011) conducted according to the anatomic pathology laboratory database, located in Joinville/SC. Both conventional cytology and liquid based cytology were included in this study. We established the ASC/SIL ratio per year and the results were compared to the ones published by CAP. Results: The ASC/SIL ratios were the same as those recommended by CAP, with an average of 1.467. Conclusion: The ASC/SIL ratio provides a simple and effective feedback to laboratories about the ASC diagnosis, indicating the need to reduce or elevate this ratio whenever necessary. |

La razón entre las tasas de ASC y de lesiones intraepiteliales escamosas (SIL, squamous intraepithelial lesion) ha sido utilizada como una herramienta de control de calidad por muchos laboratorios de citopatologia. Objetivo: Evaluar la razón ASC/SIL anual, en un servicio de anatomía patológica de gran porte durante un periodo de 10 años, comparando los datos con los valores preconizados por el Colegio Americano de Patólogos (CAP, College of American Pathologists) y averiguar su utilidad como herramienta que garantiza la calidad en la interpretación citológica de ASC en el laboratorio de Citopatología. Método: Estudio transversal y retrospectivo (2002 a 2011) realizado con base en los registros de ASC y SIL, pertenecientes a la base de datos de un servicio de anatomía patológica de Joinville/SC. Todos los exámenes fueron contabilizados, incluyendo las citologías convencionales y las citologías en medio líquido. Fue establecida la razón ASC/SIL por año y los resultados fueron comparados con los índices preconizados por el CAP. Resultados: Los índices encontrados de la relación ASC/SIL están dentro de los valores preconizados por el CAP, con un promedio de 1,467. Conclusión: La relación ASC/SIL ofrece una información simple y eficaz para el laboratorio de citopatologia acerca del uso de ASC como categoría de diagnóstico, indicando la necesidad de adecuar el índice, por aumento o diminución, según las necesidades.Introdução: Atipia de células escamosas (ASC, atypical squamous cells) é a anormalidade epitelial mais frequente nos resultados de análises colpocitológicas e corresponde essencialmente a um grupo epidemiológico de incerteza. A razão entre as taxas de ASC e de lesões intraepitelias escamosas (SIL, squamous intraepithelial lesion) tem sido utilizada como uma ferramenta de controle de qualidade por muitos laboratórios de citopatologia. Objetivo: Avaliar a razão ASC/SIL anual, em um serviço de anatomia patológica de grande porte durante um período de dez anos, comparando com os valores preconizados pelo Colégio de Patologistas Americanos (CAP, College of American Pathologists); e averiguar sua utilidade como ferramenta de garantia da qualidade na interpretação citológica de ASC no laboratório de citopatologia. Método: Estudo transversal e retrospectivo (2002 a 2011) realizado com base nos registros de ASC e SIL do banco de dados de um serviço de anatomia patológica, em Joinville/SC.
