Advanced Breast Cancer as a Sentinel Event for the Evaluation of the Breast Cancer Early Detection Program in the Midwest of Brazil

Resumo: Introduction: Breast cancer when found in its early stages, there is great possibility of cure. However, late diagnosis of the disease is still reality in several regions of Brazil. Objective: To analyze events related to diagnosis at an advanced stage of breast cancer and verify the path followed by women in health services. Method: An observational, descriptive, cross-sectional study was conducted, using the technique of investigating sentinel events in women diagnosed with advanced breast cancer, residents of the State of Mato Grosso do Sul. Qualitative data analysis was conducted according to thematic categories. Results: 12 with mean age of 47.3 years were interviewed. 58.3% of the women had mammography and breast ultrasound before cancer diagnosis and 75.0% said that the breast examination was performed on the Basic Unit Health. 58.3% women realized themselves the appearance of breast lump and the feeling of fear or ignorance was responsible for diagnosing advanced stage; another important aspect in the delay was the low case-resolving capacity of the health service. Conclusion: There is a need to develop effective strategies that prioritize early diagnosis of breast cancer in a timely manner, offering women effective care and easy access.

Sin embargo, el diagnostico tardio de la enfermedad sigue siendo la realidad en varias regiones de Brasil. Objetivo: Analizar los eventos relacionados con el diagnostico en etapa tardía de cáncer de mama y determinar la trayectoria recorrida de las mujeres en los servicios de salud. Método: Se realizó un estudio observacional, descriptivo, transversal, através de la técnica de investigación de los eventos centinela en mujeres diagnosticadas con cancer de mama avanzado, residentes en el Estado de Mato Grosso do Sul. Los datos fueron analizados descriptivamente y en el enfoque cualitativo se hizo un análisis del contenido según las categorías temáticas. Resultados: Se entrevistó a 12 mujeres con una edad promedio de 47,3 años, el 58,3% de las mujeres tuvo una mamografía y ecografía de la mama antes del diagnóstico de cáncer y el 75% dijo que el examen de mama se realizo en la Unidad Básica de Salud. En 58,3%, las propias mujeres se dieron cuenta de la aparición del tumor en la mama y la sensación de miedo o de desconocimiento se encargaron del diagnostico en una fase avanzada, otro aspecto importante de la demora fue la ineficiencia de los servicios de salud. Conclusión: Se vio la necesidad de desarrollar estrategias efectivas que prioricen el diagnostico precoz del cáncer de mama en el momento oportuno y ofrecer a las mujeres servicios resolutivos y con fácil acceso. Introdução: O câncer de mama quando descoberto em sua fase inicial, há grande possibilidade de cura. Entretanto, o diagnóstico tardio da doença ainda e realidade em várias regiões do Brasil. Objetivo: Analisar os eventos relacionados ao diagnóstico em estádio avançado do câncer de mama e verificar a trajetória percorrida pelas mulheres nos serviços de saúde. Método: Foi realizado um estudo observacional, descritivo, transversal, por meio da técnica de investigação de eventos sentinela em mulheres com diagnostico de câncer de mama avançado, residentes no Estado do Mato Grosso do Sul. Os dados foram analisados de forma descritiva e, na abordagem qualitativa, realizou-se a análise de conteúdo segundo as categorias temáticas. Resultados: Foram entrevistadas 12 mulheres com média de idade de 47,3 anos, 58,3% das mulheres fizeram a mamografia e a ultrassonografia mamária antes do diagnóstico de câncer e 75,0% afirmaram que o exame clínico das mamas era realizado na Unidade Básica de Saúde. Em 58,3%, as próprias mulheres perceberam o aparecimento do nódulo na mama e o sentimento de medo ou desconhecimento foram responsáveis pelo diagnóstico em estádio avançado; outro aspecto importante no atraso foi a baixa resolutividade do serviço de saúde.
