Advanced mucoepidermoid carcinoma

Resumo: The present study is based on a series of 18 cases of mucoepidermoid carcinoma of salivary glands. The histologic study of the biopsies and surgical specimens was perfomed by two pathologists, who confirmed the diagnosis in 16 pacients with mucoepidermoid carcinoma, which were classified as follows: 4 low-grade, 4 intermediate-grade and 8 high-grade of malignancy, based on the morphologic criteria described by Auclair & Ellis (1991), with the specimens stained by H.E. and Mayer’s mucicarmin. In the remaining 2 cases, the initial diagnosis was maintained without the histological grading. In this study we reviewed the cases of mucoepidermoid carcinoma. The parotid gland was the most frequently affected. Disease control and survival were related to the degree of differentiation, according to the distribuition by sex, age, anatomic location, clinical stage and rate of recurrence. The parameter T of the TNM classifcation, was also correlated with survival: stage T3 presented survival of 41,6% for five years and the stage T4, of 17,0%. A reduction of the lenght of survival was also observed when the N parameter increased from stage N0 to N3, survival for three years was 0%.

Os estudos histológicos realizados por dois patologistas evidenciaram 16 pacientes portadores de tumores de glândulas salivares maiores e menores com o diagnóstico de carcinoma mucoepidermóide, sendo 4 de baixo grau, 4 de grau intermediário e 8 de alto grau de malignidade. Nos 2 casos restantes foram mantidos os diagnósticos iniciais sem a realização de subclassificação. Neste estudo revisamos os casos de carcinoma mucoepidermóide mostrando que a glândula parótida foi a mais afetada. Quanto aos fatores que exerçam influência no controle da doença e na sobrevida, foram analisados o grau de diferenciação em relação à distribuição por sexo, idade, localização anatômica, estádio clínico e taxa de recidiva. A correlação do parâmetro T da classificação TNM, com a sobrevida mostrou que os casos do estádio T3 apresentaram sobrevida em cinco anos em torno de 41,6% e do estádio T4,17,0%.
