Functional Capacity and Quality of Life in Women after Mastectomy

Resumo: The objective of this study was to evaluate the functionality of the upper limb, quality of life and daily life activities of women who have undergone mastectomy. Twenty women, volunteers who had undergone mastectomy were assessed. The evaluation consisted of anamnesis, a functional assessment that included range of motion (goniometry), muscle strength (hand evaluation), and completion of the Study's Short Form-36 and Frenchay Activities Index questionnaires. The study found a significant decrease in muscle strength in the abduction (p=0.01), flexion (p=0.01) and external rotation (p=0.008) movements of the shoulder, as well as in the range of motion of the lateral rotation (p=0.02), abduction (p=0.004) and flexion movements of the shoulder (p=0.02) on the same side as the surgery. The Study's Short Form-36 showed greater impairment in the areas of limitation by physical aspects (52.50), pain (55.45) and vitality (54.75). The Frenchay Activities Index questionnaire values showed a decrease in activities such as preparing food, washing clothes, heavy duty chores, nearby shopping, driving or travelling by bus. Summing up, there was a decrease in the range of motion and muscle strength in the movements of lateral rotation, flexion and abduction of the shoulder, which, associated with complaints of shoulder pain, may have promoted a negative impact on the Quality of Life, though it is not associated with the decrease in heavy duty chores evaluated by the Frenchay Activities Index.

Se evaluaron 20 mujeres, voluntarias, sometidas a la mastectomía. La evaluación consistió en anamnesis, la evaluación funcional que incluye un rango de movimiento (goniometría), la fuerza muscular (evaluación manual) y aplicación de cuestionarios Study's Short Form-36 [Formulario Corto del Estudio-36] y Frenchay Activities Index [Índice de Actividades de Frenchay]. El estudio constato una disminución significativa de la fuerza muscular en los movimientos de abducción (p=0,01), flexión (p=0,01) y rotación lateral (p=0,008) del hombro, y del rango de movimiento de rotación lateral (p=0,02), abducción (p=0,004) y flexión (p=0,02) del hombro en el mismo lado de la cirugía. El Study's Short Form-36 mostro mayor deterioro en las áreas de limitación en los aspectos físicos (52,50), dolor (55,45) y vitalidad (54,75). El cuestionario Frenchay Activities Index mostro disminución en los valores en actividades tales como preparar alimentos, lavar la ropa, servicios pesados de casa, compras locales y conducir coches o viajar en autobús. En conclusión, ocurrió la disminución de la amplitud de movimiento y fuerza muscular en el movimiento de rotación lateral, flexión y abducción del hombro que relacionada con quejas de dolor en el hombro, puede haber promovido un impacto negativo en la Calidad de Vida, pero que no está relacionada con la diminución de actividades pesadas evaluadas por Frenchay Activities Index.O objetivo deste estudo foi avaliar a funcionalidade do membro superior, a Qualidade de Vida e as atividades de vida diária de mulheres submetidas a mastectomia. Foram avaliadas 20 mulheres, voluntarias, submetidas a mastectomia. A avaliação consistiu de anamnese, avaliação funcional, que englobou a amplitude de movimento (goniometria), forca muscular (avaliação manual) e aplicação dos questionários Study's Short Form-36 e Frenchay Activities Index. O estudo constatou que houve diminuição significativa da forca muscular nos movimentos de abdução (p=0,01), flexão (p=0,01) e rotação lateral (p=0,008) de ombro, e da amplitude de movimento de rotação lateral (p=0,02), abdução (p=0,004) e flexão (p=0,02) de ombro no membro homolateral a cirurgia. O Study's Short Form-36 mostrou maior comprometimento nos domínios de limitação por aspectos físicos (52,50), dor (55,45) e vitalidade (54,75). O questionário Frenchay Activities Index apresentou diminuição dos valores nas atividades como preparar comida, lavar roupas, serviços pesados de casa, compras locais e dirigir ou viajar de ônibus.
