Cultural Aspects Involving the Patient Diagnosed with Prostate Cancer: a Study in the Community

Resumo: Introduction: Prostate cancer is matter of World Public Health. Cultural issues involved in the prostate cancer diagnosis are closely related to the social feeling of the masculinity hegemony which is characterized as the main factor for the low number of men who seek health centers. Objective: Thus, this study intends to understand the cultural aspects involved in the diagnosis of prostate cancer in men from the south of the state of Espirito Santo. Method: An exploratory randomized search in the Oncology Center at the Evangelical Hospital of Cachoeiro de Itapemirim was performed. The tolerable sample was calculated, defining 170 patients to whom a structured questionnaire was applied. All the procedures were approved by the Ethics Commitee of São Camilo University Center (ES) (Protocol 205). Results were demonstrated in terms of absolute and relative frequency, and the statistical significance was p<0.05. Results: Most of the evaluated patients were illiterate (53.2%) and above 60 years old (54.8%), living in the town of Cachoeiro de Itapemirim (50%). Among them, the majority asserted to have unconditional support from their families (90.3%) and to face the diagnosis with serenity (77.4%). However, 51.6% of them reported a worsening in their sexual lives after diagnosis. It can be highlighted that 19.4% of the patients only sought medical assistance after feeling pain, and 19.4% when they had urinary disorders. Conclusion: Cultural and social aspects such as masculinity, education, family, beliefs and myths involving prostate cancer directly influence the way patients deal with the diagnosis in the south of the state of Espirito Santo.

Las cuestiones culturales implicadas en el diagnóstico de cáncer de próstata están directamente relacionadas al sentimiento social de la hegemonía de la masculinidad que está caracterizada como el principal factor de la poca búsqueda de los varones por los servicios de salud. Objetivo: Por lo tanto, el objetivo de este estudio fue conocer a los aspectos culturales presentes en el diagnóstico de cáncer de próstata en los varones del sur de Espírito Santo. Método: Se realizó una investigación exploratoria al azar en el Centro de Oncología del Hospital Evangélico de Cachoeiro Itapemirim Evangélica. 170 pacientes, a los cuales fue entregue un cuestionario estructurado, fueron elegidos en muestra tolerable. Todos los procedimientos fueron aprobados por el Comité de Ética del Centro Universitario São Camilo (ES) (Protocolo 205). Los resultados fueron presentados en frecuencia relativa y absoluta, y la significación estadística fue de p <0,05. Resultados: La mayoría de los pacientes evaluados era analfabeta (53,2%), mayores de 60 años (54,8%), que viven en Cachoeiro de Itapemirim (50%). De estos, la mayoría reportó contar con el apoyo incondicional de la familia (90,3%) y que reaccionan con tranquilidad frente al diagnóstico (77,4%). Sin embargo, el 51,6% reportó un deterioro en la vida sexual tras el diagnóstico. Es importante resaltar que el 19,4% de los pacientes buscaron a un médico sólo tras sentir dolor, y el 19,4% cuando tuvieron algún problema urinario. Conclusión: Los aspectos culturales y sociales, tales como: la masculinidad, la educación, la familia, las creencias y los mitos que rodean al cáncer de próstata influyen directamente en cómo los pacientes viven con el diagnóstico de cáncer de próstata en el sur de Espírito Santo.  Introdução: O câncer de próstata é um problema de Saúde Pública Mundial. As questões culturais envolvidas no diagnóstico do câncer de próstata estão relacionadas diretamente ao sentimento social de hegemonia da masculinidade que se caracteriza como principal fator da pequena procura dos homens aos serviços de saúde. Objetivo: Dessa forma, o presente trabalho teve o objetivo de compreender os aspectos culturais envolvidos no diagnóstico de neoplasia de próstata no homem do Sul do Espírito Santo. Método: Foi realizada uma pesquisa exploratória randomizada, no Centro de Oncologia do Hospital Evangélico de Cachoeiro de Itapemirim. Foi calculada a amostra tolerável, definindo 170 pacientes nos quais se aplicou um questionário estruturado. Todos os procedimentos foram aprovados pelo Comitê de Ética do Centro Universitário São Camilo (ES) (Protocolo 205). Os resultados foram demonstrados em frequência relativa e absoluta, e a significância estatística foi p<0,05. Resultados: A maior parte dos pacientes avaliados era analfabeta (53,2%), com idade superior a 60 anos (54,8%), residente em Cachoeiro de Itapemirim (50%). Destes, a maioria relatou ter apoio incondicional da família (90,3%) e encarar com tranquilidade o diagnóstico (77,4%). Contudo, 51,6% relataram piora na vida sexual após o diagnóstico da doença. Ressalta-se que 19,4% dos pacientes só procuraram o médico depois de sentirem dores, e 19,4% quando apresentaram algum problema urinário.
