Enhanced Recovery After Surgery in Oncologic Patients: The Role of the Physiotherapist

Resumo: Due to the increase of the global cancer incidence, the number of patients admitted for cancer surgery has risen as well. Strategies to optimize postoperative recovery (fast-track surgery) are recommended, and physiotherapists play a key role in health teams caring for surgical patients. Post-operative recovery optimization protocols such as Enhanced Recovery After Surgery (ERAS) and the ACERTO Project (acceleration of total post-operative recovery) include pre-, peri- and post-operative management, with the following elements: information and pre-habilitation, abbreviation of fasting and early reintroduction of food, adequate prophylaxis and correct management of symptoms, reduction of fluids and non-colon preparation, minimally invasive procedures, and early mobilization. For individuals with cancer, the acceleration of the physical and functional recovery after surgery is essential to enable the patient to undergo adjuvant therapies within the recommended therapeutic window. These protocols address the major role physiotherapists play, especially in post-surgery pre-habilitation and early mobilization.

Se recomiendan estrategias para optimizar la recuperación posoperatoria (fast-track surgery), desempeñando el fisioterapeuta un papel fundamental dentro de los equipos de salud que atienden al paciente quirúrgico. Los protocolos de optimización de la recuperación postoperatoria como Enhanced Recovery After Surgery (ERAS) y el Proyecto ACERTO (aceleración de la recuperación posoperatoria total) incluyen un manejo pre, peri y postoperatorio, con los siguientes elementos: información y prehabilitación, abreviación del ayuno y reintroducción temprana de alimentos, profilaxis adecuada y manejo correcto de los síntomas, reducción de líquidos y no preparación del colon, procedimientos mínimamente invasivos y movilización temprana. En personas con cáncer, acelerar la recuperación física y funcional del paciente después de la cirugía es extremadamente importante, para que el paciente pueda someterse a terapias adyuvantes dentro de la ventana terapéutica recomendada. Entre los aspectos de estos protocolos, el fisioterapeuta realiza un papel importante, especialmente en la prehabilitación y movilización temprana después de la cirugía.  Com o aumento da incidência mundial de câncer, a quantidade de pacientes que se internam para realização de cirurgia oncológica também tem aumentado. Medidas de otimização da recuperação pós-operatória (fast-track surgery) são preconizadas, com um papel fundamental do fisioterapeuta dentro das equipes de saúde que prestam assistência a pacientes cirúrgicos. Protocolos de otimização da recuperação pós-operatória como o do Enhanced Recovery After Surgery (ERAS) e do Projeto ACERTO (aceleração da recuperação total pós-operatória) incluem manejos pré, peri e pós-operatórios, tendo como elementos: informações e pré-habilitação, abreviação de jejum e reintrodução precoce da alimentação, profilaxias adequadas e manejos corretos de sintomas, redução de fluidos e não preparo de cólon, procedimentos minimamente invasivos e mobilização precoce. Em pessoas com câncer, agilizar o restabelecimento físico e funcional do paciente após a cirurgia é extremamente importante, de forma a deixar o paciente apto a realizar as terapias adjuvantes dentro da janela terapêutica preconizada.
