Resumo: | From August 1986 to February 1987, three patients were treated with VP-16 - 1,800mg/m2, continuous infusion over 26h on day -6, followed by Ciclophosphamide (EDX) 50mg/kg/d on days -5, -4 and -3 and total body irradiation (TBI) - 1.000cGyin 5/200cGy fractions on days -2, -1 and day 0. Bone marrow transplantation was performed on day 0 utilizing HLA compatible donors. The study included 3 patients: 1 patient with acute lymphoblastic leukemia (ALL) in 1st marrow relapse; 1 patient with chronic myelogenous leukemia (CML) in 2nd chronic phase and 1 patient with ALL in 2nd remission. The age varied between 4 and 29 years. The most significant toxicity of this regimen was severe mucositis that occurred in all 3 patients. They all developed mouth ulcerations and one patient developed hemorrhagic diarrhea with a volume greater than 2/d. The picture resolved in three weeks in all patients. One patient had significant electrocardiographic changes but no signs of cardiotoxicity The efficacy of the regimen could be evaluated in the patient transplanted in relapse that remains in remission 210 days after transplantation. The Ph1 chromossome disappeared in the patient with CML. We conclude that VP-16/EDX/TBI is an effective regimen and well tolerated in the treatment of hematological malignancies. |

Irradiação corpórea total (ICT) 1000 cGy em 5 frações de 200cG y nos dias -2, -1 e 0. O transplante de medula óssea (TMO) foi realizado no dia O utilizando doadores HLA compatíveis. O estudo incluiu três pacientes: um paciente portador de Leucemia Linfoblástica Aguda (LLA) em 1.ª recaída medular; um paciente portador de Leucemia Mielóide Crônica (LMC) em 2.ª fase crônica e um paciente portador de LLA em 2.ª remissão. A idade variou entre quatro e 29 anos. A principal toxicidade apresentada por este regime foi mucosite severa que ocorreu nos três pacientes. Todos os três pacientes apresentaram ulcerações orais e um deles apresentou diarréia hemorrágica com volume superiora dois litros. Em todos os pacientes o quadro se resolveu nas três semanas que se seguiram ao TMO. Um paciente apresentou alterações eletrocardiográficas significativas sem, entretanto, ocorrerem manifestações clínicas de cardiotoxicidade. A eficácia do regime pode ser testada no paciente em recaída que permanece em remissão 210 dias pós-TMO. No paciente portador de LMC houve eliminação do cromossomo Ph1.
