Resumo: The best way to follow up treated breast câncer patients with no symptoms is still undefined. Concerning distant metastases, requesting several routine tests of which 99% will be normal stresses patients, cost a lot of money and has no proven. On the other side, an assessment of the breast, either the treated or the opposite one, with yearly mammography and physical exam is useful, and may improve survival. The article discusse this issue.

The article discusse this issue.A forma de seguimento de pacientes com câncer da mama é assunto controverso. Há enorme variação entre profissionais sobre quais exames solicitar e a periodicidade das consultas. Do ponto de vista de controle loco - regional, deve-se orientar a paciente para o auto-exame das mamas e consultas a cada 4 a 6 meses com mamografia anual, formando um conjunto de orientações consensual e eficiente. O aspecto menos definido refere-se à pesquisa de metástases hematogênicas de rotina em pacientes tratadas, curadas e assintomáticas. Este assunto tem sido recentemente discutido com opiniões bem antagônicas. Aparentemente, a solicitação de vários exames de laboratório e radiológicos de rotina consegue fazer diagnósticos de falha um pouco mais precocemente, mas não há mudança na sobrevida. A solicitação destes exames de rotina tem um custo alto, quando se leva em conta o enorme número dessas pacientes ansiosas e as próprias pacientes em um estudo randomizado dizem preferir um seguimento mais distante ou quando sintomáticas. Pelo menos dois estudos com pacientes assintomáticas seguidas com exames usuais de rotina, versus seguimento mais orientado por queixas, mostram que a sobrevida foi igual.
